The Emergency Plan Card is the home-health agency's Patient Emergency Plan Identification Card, kept in the app: demographics, qualifying conditions, the evacuation plan and the rest of the paper card's fields. Caregiver-role accounts maintain it, every clinical login reads it, and one button prints it. The lines typed under Diagnoses are what the Billing support report counts as conditions on file.
The wallet Medical ID reads this card (and the profile as a fallback) to prove this patient is ill in public: Show big for handing the phone to a responder, Print wallet card at wallet size. Partly: the Medical ID and the Fill from her doctor's records button appear only when Doctor's Records is switched on for the deployment; until then the page says Not set up yet.
