How it connects
Her chart, on her terms.
Epic (MyChart) and eClinicalWorks (healow) connections are built and registration with each vendor is in progress. Every connection is read-only into the family's app, with the patient's or proxy's authorization.
Patient-authorized
She — or the family member with proxy access — signs in on the hospital's own portal (MyChart, healow) and approves ExpressAgain there. We never ask for portal passwords.
Read-only
The chart flows into the family's app. Nothing the family types is written into the hospital's record without a separate agreement with that practice.
Encrypted
TLS on every connection; access tokens sealed at rest with AES-256-GCM under a key that lives only in our hosting environment. No PHI in logs or URLs.
Disconnect any time
One tap in the app drops the cached chart and the sealed tokens. Access can also be revoked on the portal itself (MyChart → Linked Apps; healow → Connected apps).

Every place she gets care, in one list
Connect more than one hospital and more than one practice for the same person. A medication, allergy or problem that appears at two places is one line, marked “also at…”. Each row says where it came from and when it was last refreshed.

One page the family hands to the practice: glucose readings against her own targets, blood pressure, weight, meals with sugar and carbs per day, symptoms and events, aide visit notes, and the medication list as the family actually gives it. Counts and averages — never a judgment.
Print it, save it as a PDF, or share it from the phone. It is never emailed in plain text. Delivery straight into the practice's inbox by Direct Secure Messaging is next.
The care team's dashboard shows the same chart, read-only, so a visiting nurse or therapist sees what the doctor sees.

For hospitals and practices that exchange ADT (admits, discharges, transfers), ORU (results) and SIU (scheduling) feeds, we run an HL7 v2 interface engine (Mirth Connect / Open Integration Engine) and map the feed into the family's circle. For home-health agencies and facilities that want their own systems talking to ExpressAgain, the Partner API is available on request.

Early access
Families using ExpressAgain already keep the readings, meals, symptoms and visit notes a care-management team wishes it had. We are opening early-access conversations with health plans about outcomes data and a care-management workflow built on it — with the member's authorization, every time.

Security & privacy
Plainly: what we do with her chart.
- We pull data only after she or her proxy consents on the vendor's own screen, and only the read-only USCDI scopes.
- Her chart is stored under the family's own account like the rest of her record; access tokens are sealed at rest; nothing from the chart appears in logs or web addresses.
- Friends in the circle never see the chart. Family reads; only the owner and caregiver accounts can connect or disconnect.
- Business Associate Agreement: we sign a BAA with partner organizations once our vendor agreements are complete — in progress now; ask us for the timeline.
- Her data is never sold. Ever.
