A practice that follows the care
The practice enrolls the patient; the owner or a caregiver-role account taps Accept on a link that names the practice and the primary doctor. From then on the practice appears under Organizations following the patient's care on the Care Team page, and Manage ends it any time.
The readings taken at home
Blood pressure, glucose, weight and the rest go into Health Tracking as they are taken, each with who logged it. The practice's clinicians read the very same chart on their dashboard, and Copy and Print reproduce the sheet for a visit.
The papers Medicare asks for
Important papers holds the Medicare permission to share (CMS-10106) and the appointment of representative (CMS-1696), with Fill in the official form, a status for each and the signed copy uploaded; the app reminds the family 30 days before the CMS-1696 year ends.
The month, documented for the practice
On the care-team dashboard the practice logs its time and contacts; the Billing support report under Reports shows the month's minutes, contacts, consent and plain requirement checks per program, labeled Estimated reimbursement opportunity, and the practice's coder decides.
The program, in plain words
Medicare Part B may pay a doctor's practice for chronic care management for a person with two or more serious chronic conditions expected to last at least a year: a comprehensive care plan, 24/7 access for urgent needs, support when moving from one setting to another and a review of the medicines; and for transitional care management after a hospital stay, with a contact within two business days of discharge and a visit within 7 or 14 days. The practice bills; the family keeps the record the practice reads.

Each one is a contact card with a name, a role and a phone number to tap and call: the primary doctor, the cardiologist, the clinic. Log a call or visit records each call, who was on it and what was decided, and Next visit puts the appointment on the calendar with what to bring and who is driving.

Care Team helps manage the care. Home Team helps the patient at home: the aides, CNAs, LPNs and RNs on shifts, the companions, the household help. Each professional has one profile with the agreements, the documents, the pay and the schedule, and the home team's clocked hours show read-only on the practice's Billing support report under Home team hours.

Find a team member sends a questionnaire to anyone the family is considering, and the Marketplace search finds professionals by service and ZIP area. Agencies & Quotes writes a de-identified overview and the proposed schedule, sends one link, and the agencies' quotes land back in the app side by side, with no access to the record.

Home Team agreements drafts the Independent Service Agreement from the template, sends it, the professional signs, the family countersigns; an agency's own PDF goes in with its rules. One table shows who has completed the agreement, the W-9, CPR, the license, TB and the skills checklist. A professional fills and signs the W-9 in the app; the family sees it on file with the last four digits, and the owner's reveal for a 1099 is recorded every time.

The family writes the Start of shift and End of shift items. An item can be verified from the day's data, such as vitals logged, medications given or a shower logged, or by a photo the professional takes before it counts. A required item left undone holds that day's pay for the family's review, and hours worked are always owed.

The clock becomes the timesheet. Each professional, or the agency office, sends an invoice for the pay period; the family approves it, and Pay now shows their chosen method: Zelle, Cash App, direct deposit, a check, a bank transfer or a wire. Mark paid records how and with what reference. Year summary lists what the family paid each professional or agency, by the day it was paid, and flags an independent professional paid at or over the IRS reporting threshold with 1099-NEC likely due.
