For case managers

A great resource for your clients. And you see how it goes.

For case managers, social workers and care coordinators. Sit with the patient, read one screen together, and send the family their link. The family owns the account: a voice when words are hard, the medications and the care plan in one place, the family circle and the Care Team around them. You see the referral, the claim, the sign-ins and how active the family has been.

Referring is free. The family owns the account.

What the patient and family get

One plan, a voice, the feed and the Care Team.

ExpressAgain runs in the web browser on a phone, tablet or computer; there is nothing to install. The patient is named by name on every screen, and the people who help at home are professionals.

  • One plan

The patient's day plan, medications with every dose and every change, appointments, the emergency card and the papers, in one place the family and the care team share. Setup asks what the patient needs help with and opens those pages first.

  • The voice

Tap a phrase and the app says it aloud; build a sentence word by word; practice out loud. The voice can be the patient's own, recorded from a short sample, so what the family hears is still them.

Tap any phrase to speak it →
  • The feed

The family's own feed for the day, photos and messages, and the IDC Team feed where every reading, dose and note the care team logs is one line with initials and the time.

    The IDC Team feed →
  • The Care Team

Doctors, nurses, therapists and clinics on one page, tap-to-call, with logins for the clinicians who need the clinical picture. With the family's yes, the case manager who referred them sits here too.

How referring works

Three steps, in one sitting.

  1. Sit with the patient and open Refer a family

One screen, written to the patient in large type: as your case manager, I want to help you communicate again and manage your care. You read it together, with your name on it.

  2. Type the patient's first name and the family's email or cell

The family gets one link. The message names you and ExpressAgain, never a diagnosis and never a detail about the patient.

  3. The family claims the account, and you see what happened next

The link opens the care plan. The family claims the account, decides how to use it, and chooses whether you stay on the Care Team. My referrals shows each step.

The arrangement

Referring is free. The family owns the account.

You refer; you never pay and never collect. Your account exists to refer families and to see what happened next, and it enrolls no patients of its own.

The family claims the account and it is theirs: who they invite, what they share, and whether you stay on their Care Team. Our own team follows up with each family about the plan that fits them.

What you'll see afterwards

My referrals: the day you referred them, the claim, the sign-ins, and how active the family has been.

Counts, never contents. What a family logs stays with the family unless they keep you on the Care Team; then you read it there, on their dashboard, the way every care-team login does.

  • Your referrals

Every family you referred, with the patient's first name and the day you referred them. An organization administrator sees every member's, with the referrer's name.

  • Claimed

The day the family claimed the link, or Link sent, not claimed yet. Then the family's last sign-in.

  • Active

How many of the last 30 days the family used the app, and what was logged as counts: symptoms, meals, doses, feed posts. Never what they said.

  • The Care Team view, when the family says yes

With the family's yes you sit on the Care Team and open their dashboard: the feed, the symptoms, the medications, the same view every care-team login reads.

One team, one plan

Built for the interdisciplinary care team

ExpressAgain is an integrated patient care, communication, and care management platform built for people who can think, read and remember clearly but struggle to speak, as well as people who can speak for themselves but have no safe, trusted way to show whether the care they're receiving is the care they're owed.

As an all-in-one platform, it brings patients together with the families, caregivers, nurses, therapists, physicians and agencies who care for them, so everyone works from one plan, one schedule and one shared record, from the hospital to home.

The same day, the same medications, the same notes, and the same record of who did what and when.

The patient in the middle, with families, caregivers, nurses, therapists, physicians and agencies in boxes around the patient, an arrow between each box and the patient, and a ring through all six boxes that says One plan, One schedule, One shared record. The patientone person, one planFamiliesthe daughter, thesiblings, thefriends who show upCaregiversaides, CNAs andcompanions in thehome, hireddirectly or sent byan agencyNursesRNs and LPNs onvisits, shifts andmedications, withthe visit noteTherapistsspeech, physicaland occupational, with exercises topractice at homePhysiciansMDs, DOs, NPs andPAs, reading whatchanged since thelast visitAgenciesthe office thatstaffs, schedulesand billsOne plan · One schedule · One shared record

The patient is in the middle, and six kinds of people care for that one person. Every arrow runs between a box and the patient, and the ring runs through all six:

  • Families: the daughter, the siblings, the friends who show up.
  • Caregivers: aides, CNAs and companions in the home, hired directly or sent by an agency.
  • Nurses: RNs and LPNs on visits, shifts and medications, with the visit note.
  • Therapists: speech, physical and occupational, with exercises to practice at home.
  • Physicians: MDs, DOs, NPs and PAs, reading what changed since the last visit.
  • Agencies: the office that staffs, schedules and bills.

All of them work from one plan, one schedule and one shared record: the day plan the family writes is the one the aide follows and every care-team login reads.

Hospice, palliative care and community paramedic clinicians join the care team the same way, with a code from the family.
