ExpressAgain is an integrated patient care, communication, and care management platform built for people who can think, read and remember clearly but struggle to speak, as well as people who can speak for themselves but have no safe, trusted way to show whether the care they're receiving is the care they're owed.
As an all-in-one platform, it brings patients together with the families, caregivers, nurses, therapists, physicians and agencies who care for them, so everyone works from one plan, one schedule and one shared record, from the hospital to home.

From the hospital to home.
The same record follows the patient along this path. Each station is a part of the app that exists today.
- The hospital stay. Communicate from the bed: tap a phrase and it is spoken aloud, with ICU and after-surgery phrases, Medical Words and the emergency phrases, in the browser with nothing to install. ICU and a breathing tube
- Coming home. Coming Home holds the discharge checklists and the therapy team's home recommendations, each marked Done, In progress or Need help. For discharge programs
- The Home Team. The professionals in the home work from the day plan, the shift checklists, the Visit Flow Sheet and Clock in and Clock out, each with one profile for agreements, documents, pay and schedule. For independent professionals
- The care-team dashboard. Clinicians join with a code from the family and land on Since your last visit; therapists assign exercises, nurses write visit notes, and every login reads the same day plan. For clinicians
- The doctor's view. A physician login gets the doctor's card: vitals and glucose against the family's targets, doses, symptoms, ER and hospital events, the team's notes and therapy progress, and a weekly visit email. For practices

The record
One record, many hands.
The same day, the same medications, the same notes, and the same record of who did what and when.
Every hand writes to the same record, and every line on it says who did what and when.
- The patient. Spells a note on the Letters board and taps Share: it goes to family and friends and to the care team.
- The aide. Fills the Visit Flow Sheet, ticks the day plan's steps and logs a reading with Quick log, each marked with who and when.
- The nurse. After a home or video visit, writes a structured visit note from the visit's row; every care-team login reads it under Notes, and the family under Appointments when shared.
- The therapist. Assigns exercises that appear on the patient's Practice page, and files home recommendations that land on Coming Home.
- The family. Writes the day plan, books the appointments with the ride and who goes along, and posts the daily note to the feed.
- The doctor. Reads Since your last visit: what changed since the last logged visit, as counts and ranges against the family's targets, never a diagnosis.

- What is an interdisciplinary care team?
- A group of different health professionals who work closely with the patient and family to create a single, shared treatment plan. ExpressAgain is where that whole team cares for one patient together.
- Who is on the Care Team, and who is on the Home Team?
- Care Team helps manage the patient's care: doctors, nurses, therapists, hospice and palliative clinicians and the community paramedic. They join with a code from the family at the IDC Team sign-in and get their own dashboard, and every care-team login reads the patient's day plan. Home Team helps the patient in the home: the professionals on shifts, aides, CNAs, LPNs, RNs and companions, each with one profile that holds contact details, agreements, documents, pay and schedule.
- What does the patient use ExpressAgain for?
- To be heard. Tap or highlight a phrase anywhere in the app and it is said out loud, in a clear built-in voice or in the patient's own cloned voice; the Letters board spells a word letter by letter and Share sends it as a note to family, friends and the care team. It runs in the web browser on a phone, tablet or computer, with nothing to install.
- Does ExpressAgain replace the hospital's chart?
- No. The hospital keeps its chart. Doctor's Records is built to show the chart from Epic and eClinicalWorks read-only, with the patient's or a proxy's authorization, next to what the family tracks at home, and to hand the doctor a weekly home summary; it switches on for a deployment once each vendor's registration is approved.
- Is communicating free?
- Yes. Communicating is never behind a paywall: tap to speak, the phrase boards, the sentence builder, Letters, the Phrase Library, lessons, games, the emergency screen and the built-in voice are on the Free plan. The plans page describes the rest.
