For health plans and Medicare Advantage
A voice for members who cannot speak, and support for the families caring for them at home.
After a stroke, with aphasia, a brain tumor or ALS, the member taps a phrase and the phone or tablet says it out loud. The family, the Care Team and the Home Team keep one record of the days at home. Your plan can sponsor a member's access, so the family is never asked to pay.
30 minutes on Zoom with our founder. Nothing to prepare.

The gap
The member cannot answer the call. The family carries the care alone.
Members who cannot speak on the phone
After a stroke, with aphasia, a brain tumor or ALS, a member who can think and understand may not be able to answer a care manager's call, say where it hurts or ask a question.
Outreach reaches a voicemail
The plan's call goes to a voicemail, or to a family member who has to relay everything second hand, with nothing written down.
Families carry the care at home
A spouse, a daughter or a neighbor keeps track of the meals, the medications, the appointments, the rides and the professionals who come to the house, often on paper and in text threads.
Between visits is invisible
A fall, missed doses, a high reading or a trip to the emergency room reaches the plan late, if at all.
The outcomes you're measured on
Built for lower cost of care, fewer avoidable admissions and higher star ratings.
Lower cost of care
Daily readings, and care alerts against the doctor's targets
The family and the Home Team log glucose, blood pressure and other vitals, weight and symptoms at home every day. With care alerts on, every reading reaches the member's Care Team as one plain line, and a reading outside the targets from the member's doctor goes out at once, not at the next visit.
Targets and care alerts →Fewer avoidable admissions and readmissions
The discharge handoff, the follow-up appointment and the home log on one record
A practice or hospital enrolls the member at discharge, the family books the follow-up visit where the Care Team sees it, and the weeks at home are logged on the same record. Readmissions and transitions of care are quality measures plans are rated on.
Appointments →Medication adherence
Dose logs: given, missed or refused, with who and when
Every dose is logged from the member's day or by the Care Team, as given, skipped, refused, missed or late, stamped with who logged it and when, and counted in the Medications report. Medication adherence is part of how Medicare Advantage plans are rated.
Prescriptions and the dose log →Higher star ratings
A member who cannot speak can answer the care manager
The member taps a phrase, builds a sentence or spells a word on the Letters board, and the phone or tablet says it out loud. A member who cannot speak can answer the care manager's questions and take part in their own care.

A pilot measures these with you, in your own members.

A way to be heard, and one record of the days at home.
ExpressAgain runs in the web browser on a phone, tablet or computer; there is nothing to install. This is what a member and the family use today.
The member's own words, out loud
Tap a phrase and it is spoken out loud. Quick Speak tiles hold what is needed most, the sentence builder puts a sentence together piece by piece, a Letters board spells a word, and a Type tab speaks anything typed. There is no timer. Communicating is never behind a paywall for the member.
Tap any phrase to speak it →The member's own voice
A short recording becomes a cloned voice that sounds like the member. Until it is ready, a clear built-in voice speaks, so the member is never without one.
The cloned voice →Emergency phrases and SOS
Emergency phrases spoken with one tap, pain and where it hurts, and the SOS card: I need help now. Please come now. This is an emergency.
The Emergency page and SOS →The family circle and the daily note
The family invites family and friends with personal links, each with its own role. Every evening the app writes up the day from what was logged, by fixed rules and never by a model, and the family reviews it before it posts, so the circle hears how the day went.
The daily note →The home log
Glucose, blood pressure and other vitals, weight, sleep, meals, the bathroom, symptoms and events including falls and swelling, and personal care with how the member moved. Every entry carries who logged it. The log records and displays; it never judges a value or recommends treatment.
Health Tracking →The Care Team and the Home Team on one record
The Care Team helps manage the member's care: doctors, nurses, clinics, therapists, the hospital. The Home Team helps the member at home: aides, CNAs, nurses on shifts, companions and household help. A reading anyone logs lands on the same chart, with who logged it.

What the plan gets
Sponsorship, the family's consent and the Partner API, today.
All of it is live today. Anything else your plan needs is scoped with each plan in the pilot, and the two cards at the end say what is not built.
Sponsored access for your members
Your plan pays for a member's access, and the family is never asked to pay while the sponsorship runs. A sponsored circle is on the Family plan, and the app reads Covered by your plan's name. Our team records each sponsorship on the member's circle today; there is no plan portal yet.
Sponsorship, in plain words →The health plan pays first
When a practice or hospital also enrolls the member, your plan's sponsorship pays for base access first. The practice covers only the care management and reports it uses to follow the member.
Who pays, in order →A grace period for the family
If a sponsorship ends, the circle keeps its access for a grace period, and nothing the family recorded is ever taken away. After that, the family can continue on its own plan.
When a sponsorship ends →Member references kept sealed
A member's coverage identifier is sealed at rest; only its last 4 characters are ever readable. The family sees your plan's sponsorship at the top of Insurance & Devices, read-only.
Insurance & Devices →Sharing only with the family's consent
A family can share with your plan by typing its six-character share code, choosing exactly what your plan may read or add, and revoking it at any time. Nothing is shared until the family says so.
Share with an organization →The Partner API
For the members whose families shared with your plan: read what is recorded at home and push readings and documents back, only the scopes the family chose, with every call logged. Keys are issued after a signed BAA.
Partner API documentation →
Plan-level reporting
There is no plan dashboard today. What your plan receives at the program level is scoped with each plan in the pilot and written into the agreement.
How a pilot works →Reaching eligible members
There is no eligibility-file import today. How eligible members hear about ExpressAgain and join is scoped with each plan in the pilot.

How plans work with us
From a pilot to a program for your members.
Ways to work with us
Health Plan Pilot
A pilot with the program scope, the member population and what we measure agreed together.
Enrolled Member program
Set per enrolled member: the members who join under your plan's sponsorship.
Eligible Population program
Set across your eligible population, whether or not each member has joined yet.
Annual agreement
An annual minimum agreement for your plan's program.
Health plan pricing is set with each plan; it is not published.
How a pilot works
From a briefing to your first members at home.
A 30-minute briefing
We walk through what a member and the family use at home, how a health plan sponsorship works today, and what a pilot could look like for your plan.
Agree the scope, the population and what we measure
Together we agree the pilot's scope, the member population it serves and what we measure together, then sign the agreement. We sign a Business Associate Agreement with partner organizations once our vendor agreements are complete; that is in progress now, so ask us for the timeline.
Launch to members
Families start in the web browser on a phone, tablet or computer; there is nothing to install. Our team records your plan's sponsorship on each participating member's circle, in the way the scope sets out.
Review the results together
At the points agreed in the scope, we review what the pilot measured with your team and decide the next step: an Enrolled Member program, an Eligible Population program or an annual agreement.
What a pilot measures with you
Chosen together in the scope, in your own members. For example:
- Caregiver strain in the families carrying the care
- Avoidable ER visits after a stroke
- Members who can't speak for themselves on a call with your care managers
- Medication adherence from the dose log

What clinicians say
From the pilot practice.
These are clinicians at the physician practice piloting ExpressAgain.
This is very helpful.
This is all in one. Convenience is everything, and this is super convenient.
Why we built it
I built ExpressAgain for my mother, Debbie. Five lesions on the left side of her brain took her speech, not her mind. She still has so much to say — now she has the tools to say it.

One team, one plan
Built for the interdisciplinary care team
An interdisciplinary care team is a group of different health professionals who work together closely with the patient and family to create a single, shared treatment plan. ExpressAgain is the platform where all of those parties care for one patient together: the same day, the same medications, the same notes, and the same record of who did what and when.
