The Care Team is the people who help manage the patient's care — doctors, nurses, clinics, therapists, the hospital. They come first on the Care Team page, and the ID Care Plan group in the menu is their shared clinical picture (ID is short for interdisciplinary).
The Home Team is the professionals who help her in her home — aides, CNAs, LPNs and RNs on shifts, companions, household help. Each has one profile that holds their contact details, door code, agreements, documents, pay and schedule. The word is professional, never caregiver or staff (they may well be clinicians). The HT Care Plan group in the menu is where the family finds, hires, pays and manages them.
