What is an interdisciplinary care team?

Different health professionals — doctors, nurses, therapists, hospice and palliative clinicians, paramedics — working closely with the patient and family on one shared treatment plan, instead of each keeping a separate one. In the app, the ID in ID Care Plan is short for interdisciplinary: that menu group is the Care Team's shared clinical picture, and every care-team login, whatever its discipline, can read the day's plan there. The Care Team page lists the doctors, nurses, clinics and therapists who help manage the patient's care; the Home Team is the professionals who help the patient at home.
