  1. Support communities on MeetMindz

    • Families
    • Nonprofits

Families can talk with other families going through the same thing, in groups on MeetMindz, a separate community site with its own sign-up. There is a group for brain tumors and glioblastoma, aphasia and stroke, ALS and Parkinson's, dementia, head, neck and throat cancer, ICU and recovery, hospice and palliative care, and family caregivers. Nothing from ExpressAgain is shared with MeetMindz.

How to use it· 4 steps
    1. Open the menu and the Family & Friends group.
    2. Tap Support communities (right after Comeback Stories).
    3. Read the list of groups and the separate-site line.
    4. Tap Join on MeetMindz. It opens in a new tab, where you sign up on MeetMindz itself.
Open it Read the help articleIn the app: Support communities
  2. Home visits: Log this visit, and in-home assessment requests

    • EMS
    • Practices
    • Agencies

A clinician who visits the home logs the visit from the field view: when, any vitals taken, and the findings in plain words. It is shared with the family and everyone on the care team, and the patient's record shows a Last visit card. A practice can ask a partner for an in-home assessment (Routine, Same day or Urgent); the partner accepts and assigns someone, or declines with a reason, and the visit they log completes the request. A partner's own system can also send visits through the Partner API, whose keys are issued after a signed BAA.

How to use it· 5 steps
    1. Practice: on Patients, tap Request an in-home assessment on the patient's row, type the reason, pick How soon, who you would like to visit and the partner, then tap Send the request.
    2. Partner: open Caseload. Open requests sit on top; tap Accept and pick the person, or Decline with a reason.
    3. On the day: tap Field view on the patient's row (or Log this visit on the accepted request).
    4. Tap Log this visit, set the date and time, add the vitals you took and the Findings, then tap Save the visit.
    5. The request shows Findings sent for the practice, and the family sees the visit on the feed.
Open it Read the help articleIn the app: Organization panel (Caseload for an EMS service)
  3. Snap the bottle: read a prescription label into a medication

    • Families
    • Professionals

Take a photo of a pharmacy label and the app reads it into a medication form for the family to check: the name, strength, directions, prescriber and pharmacy. Anything the label did not say clearly is marked fill in, and nothing is saved until the family confirms. A bottle already on the list offers Update the one we have. A care-team login or a Home Team professional can snap a bottle found in the home; it waits under Found in the home until the family adds it, updates the one on the list, or marks the old one set aside. The patient's name, address, birth date and phone from the label are never kept.

How to use it· 5 steps
    1. Open ID Care Plan, then Prescriptions.
    2. Tap Snap the bottle, then Take or choose a photo of the whole label, flat and in focus.
    3. Check each field; type anything marked fill in.
    4. Tap Add to the list, or Update the one we have when the medication is already there.
    5. Optional: tick Keep this photo on the medication, or Set up a taper from these directions after saving.
Open it Read the help articleIn the app: Prescriptions
  4. Appointments: Did this happen?

    • Families
    • Professionals

Once a booked appointment's time has passed on the patient's clock, the app asks Did this happen? with Done, Didn't happen or Rescheduled. The family and the professional the visit is with can answer, add what happened or why it did not, and Undo. Marked visits stay on the Schedule with a pill and no longer hold the time. When a professional marks their own visit done, Log my minutes fills in Log a contact for them.

How to use it· 4 steps
    1. Open Appointments (or the patient's Schedule) after the visit time.
    2. Under Did these happen?, tap Done, Didn't happen or Rescheduled on the visit.
    3. Optional: tap Add what happened (or Add why) and type a line.
    4. Professionals: after Done, tap Log my minutes, check the details and save, or tap Not needed.
Open it Read the help articleIn the app: Appointments
  5. EMS and community paramedicine: Caseload, crews, the field view and the partner directory

    • EMS
    • Practices

An EMS or community paramedicine service can be its own organization. Its panel reads Caseload: the patients practices referred to it, with the crew assigned to each referral. The field view is a phone-first page for one patient: allergies and DNR, current medications with today's dose, recent dose changes, the latest readings with who and when, and the emergency card. The partner directory on Settings lets an organization be listed, find a partner on ExpressAgain, and ask, accept or decline a partnership.

How to use it· 4 steps
    1. Sign in at the IDC Team sign-in with your organization email and password; the panel opens on Caseload.
    2. Administrators: on a referred patient's row, tap Add crew and pick who visits.
    3. Before a visit, tap Field view on the patient's row and read it on your phone.
    4. On Settings, open Partner directory: choose whether your service is listed, open Find a partner on ExpressAgain, and answer requests.

  6. Team tasks with a do-by date and reminders

    • Families
    • Professionals
    • Practices

A task is something one or more people on the patient's team need to do by a date: who to call, what to ask, which form to bring. Each person sees their own open tasks on their dashboard and gets a reminder a week out, the day before, and once when it is past due. Done tasks show who closed them and when. Friends never see tasks.

How to use it· 5 steps

    2. Tap Add a task, say what needs doing, and pick Who it is for.
    3. Set the do-by date (a date, or words like early December) and, if it is about an appointment, pick it.

    5. When it is done, tap Mark done; Open it again undoes that.
Open it In the app: Care Team
  7. Standing instructions from the care team

    • Families
    • Professionals

Instructions that were said out loud, like water every two hours during chemo or check blood pressure once a day, now live in one list with who said it, when, and until when. An instruction can link to the drink-water reminder or carry a daily phone reminder. The list shows on Health Tracking, the Status Board and the care dashboard, and every change is kept so an earlier version can be put back.

How to use it· 5 steps
    1. Open ID Care Plan, then Health Tracking, and open Standing instructions from the care team.
    2. Tap Add an instruction and type it in the words it was given.

  8. Show us: report a problem with a recording, from any page

    • Everyone

A Show us button sits beside the owl on every signed-in page: the family app, the care dashboard and the organization panel. It opens Show us what's wrong with the current page attached. Record the screen (on a computer), upload a video or screenshot, or paste a link, and a real person reads it. The same three ways sit under Show us (optional) on every Suggest a feature form, and you follow the status with your suggestions.

How to use it· 4 steps

  9. Which days a medication is given: cycles, weekdays, every few days, set days

    • Families
    • Professionals
    • Practices

A medication no longer has to be every day. Choose Days of the week, Every few days, On and off (cycles, such as 5 days on and 23 off for 6 cycles), Set days, or a Taper. On any other day nothing is due: no dose chips, no pop-up, no phone reminder, and the reports never count it as missed. Cycles and set days also show on Appointments for the next two months. Paused and stopped now stay in step with the dose history on every device.

How to use it· 5 steps
    1. Open ID Care Plan, then Prescriptions, and tap the medication.

A taper is a dose that steps down (or up) on set days, like every 6 hours for 14 days, then every 12 hours for 4 days, then once a day.
