Former team. At the bottom of My Care Team, Former team lists everyone whose work with you ended. Each one shows the last day and the rehire status: Not eligible for rehire, Only with review, Eligible for rehire, or Rehire status not recorded. See the note opens the reason, the note, who set it and when, and the edit history. The account owner and the circle members who manage care (the people who hire) see it; the professional, other professionals and the patient's pages never do.
Close out someone who already left. When a professional stopped coming before you ended things properly (for example, they quit after a few shifts and you have paid them out), open their profile, tap the Manage tab, and use Close out. In one step it records the last day they worked (today or earlier), Eligible for rehire?, and with No or Only with review the reason and the note. Nothing is sent to them: no notice and no final-invoice request. If their login is still active, their access ends now, their house codes are revoked, and any of their shifts after that day go back to the open board. If they were already removed, only the record is written.
Changing it later. Change on the profile's Rehire status card lets you correct the status, the reason or the note. The version before is kept in the edit history, with who set it and when, so whoever hires next can read what happened.
The note is a private business record. Write it factually. It is not a background check and is never shared with anyone outside the home or agency that wrote it.
