Where you are

Your voice changed. You did not.

When the words are slow, or gone for a while

After a stroke, with ALS, a brain tumor, head and neck cancer or a breathing tube, speaking can be the hardest part of the day. Communicate speaks for you, at your pace, with no timer.

Your care is yours to run

Your doctors, appointments, medications, readings and papers sit in one account that you own, on a phone, a tablet or a computer, in the browser.

You choose who comes in

Nobody joins without a single-use invitation tied to a named person. Family, friends and clinicians each get their own login and see only their part.

Help that answers to you

Get Help Now reaches the people you chose when you need them. I am not safe reaches outside help, privately, when the problem is closer to home.

Your voice

Say what you mean, at your own pace.

Communicate is free and never behind a paywall. Every card below is in the app today and links the Help Center article that describes it.

  • Tap a phrase and it is spoken

One tap says it out loud. There is no timer and no rush: the app waits as long as it takes.

    How it works

Communicate is your workspace. Speak holds Quick Speak tiles for what you need most, your recent and saved phrases, and your own words. Type speaks anything you type. Letters is an on-screen alphabet board: spell a word letter by letter, hear it, and Share sends it as your note to family and friends and to your care team. A 3 at a time switch shows fewer choices on calmer days.

Tap any phrase to speak it →
  • Build a message, piece by piece

Starters like I am, What about and Let's open long lists of the things a person in your place actually wants to say.

    How it works

When a ready-made phrase is not enough, Build a message puts a sentence together from starters and endings, written for being underestimated, being tired, wanting real information and having opinions about your own care. Speak the finished message, save it for later or star it.

    Build a message →
  • Your condition's phrases come first

Choose your mode in the guided setup, and its phrases open first in the Phrase Library and on Communicate.

    How it works

Aphasia and Stroke, ICU and Ventilated, ALS and Parkinson's, Head, Neck and Throat, Dementia and Senior Care, Hospice and Palliative: each mode has its own phrases, with the urgent ones tagged, and every phrase in the library stays open to you. Change mode any time from the sidebar or the Profile page.

    The Phrase Library →
  • Your own words, added by you

The names, places, shows and needs no ready-made library could know, in a group with your name on it.

    How it works

A grandson's name, your church, a favorite show: type it, or start from Call ___, I want my ___, Put on ___ or Where is ___?. As the account owner you add, edit, group and archive them yourself, and they come first in the Phrase Library and on Communicate, as Sam's words. Your speech therapist can add words too, from the care-team dashboard.

    Your own words →
  • Medical Words, in plain language

What an MRI is, why doctors use saline, what to expect, with questions you tap to ask out loud.

    How it works

Every entry says what the thing is, why doctors use it and what to expect, and offers questions to ask the team with one tap, such as Why do I need saline? It explains words, never your own case: every card points you back to your care team.

    Medical Words →
  • A voice you choose

A warm built-in voice works right away. A signed-in account adds natural voices.

    How it works

In Settings, under Voice and Accessibility, pick the voice and set the speed, pitch and volume until it sounds right; slower is often clearer. Test my voice plays a line so you can hear the choice.

    Choosing a voice →
  • Your own voice, kept

A short recording becomes a voice that sounds like you, so I love you sounds like you.

    How it works

Record yourself from Settings, or upload a recording, best made while speaking is still comfortable or before a planned surgery. Until the voice is ready, a clear built-in voice speaks, so you are never without one. The recording goes to the voice service that builds the voice and is not stored by us. Part of the Individual plan.

    Your cloned voice →
  • Your name and date of birth, ready to tap

Hospitals ask for both constantly. Enter them once and they are pinned as two phrases.

    How it works

The guided setup asks for your name and date of birth and turns them into My name is … and My birthday is …, pinned and one tap away for every nurse, every desk and every new specialist.

    The guided setup →
  • Bigger text, bigger buttons, higher contrast

Set them in the guided setup or in Settings, and the whole app reflows to match.

    How it works

Larger text, bigger touch targets, higher contrast and reduced motion, plus a simple-language preference that keeps wording short. The app is built for one-handed use, tired eyes and hard days.

Your care

Your doctors, your medications, your records, run by you.

As the account owner you hold the record. The app says your name on every label, never a pronoun: Sam's Day, Sam's Schedule, Sam's care.

  • Your doctors, one tap away

Each doctor, nurse and clinic is a card on the Care Team page with a name, a role and a phone number to tap and call.

    How it works

Add a contact at the top. Next visit puts the appointment on your calendar. Log a call or visit keeps what happened: a call you made, an office or video visit, an emergency room visit, a hospital admission and the discharge date, with who was involved and a note.

    The Care Team page →
  • A login for the clinicians you choose

Give access on a card mints a single-use code for that person's discipline.

    How it works

Speech therapist, physical therapist, occupational therapist, nurse, physician (MD / DO / NP / PA), palliative care, hospice and more. The person joins at the IDC Team sign-in and reads the day plan, your medications, your tracking and the notes. A doctor can also stay a simple contact card with no login: a person is one or the other.

    Invite a clinician with a code →
  • Appointments, with the ride and what to bring

Book each visit with where to go, what to bring, who is driving and who goes along.

    How it works

It lands on your Schedule the moment it saves, and nothing can overlap. The day before, an email reminds you, with who is driving. After the visit, Write what happened keeps your notes and the next appointment.

    Appointments →
  • Health Tracking you log yourself

Blood pressure, temperature, oxygen, pain, weight, sleep, meals and symptoms, in one chart.

    How it works

Every entry carries who logged it and when, and a symptom can carry a photo or a short video. Green and red only say inside or outside the targets you typed in: the chart records and displays, and it never judges a value or recommends treatment. The same chart is on your care team's dashboard.

    Health Tracking →
  • Medications, with a reminder when a dose is due

Your medications, allergies and pharmacies on one page, and a pop-up when a set is due.

    How it works

Enter the times from the label. When a set is due, the pop-up asks and Given now logs it. Turn on phone notifications and the reminder reaches your phone, with no medicine named on the lock screen. ExpressAgain reminds; it does not decide when a medicine is taken, and it does not check or suggest doses.

Medication schedules and the reminder →
  • My Journal and mood check-ins

One-tap check-ins that speak for you, kept beside your vitals.

    How it works

My Journal keeps how you feel, day by day. The Care Team Report puts your check-ins, observations, care events and vitals into one dated, printable document to hand to a doctor or a new specialist: your own records, exactly as written, with no scoring.

The journal and the Care Team Report →
  • Your Emergency Plan Card

Your emergency identification card, kept in the app and printed with one button.

    How it works

The paper card's fields, filled in once: who you are, your conditions, the evacuation plan and the rest. Your medications and allergies feed it, and every clinical login on your care team reads it.

    The Emergency Plan Card →
  • Your papers, and your say at the signing

Important papers lists who can speak for you, and Signing Mode gives you quiet answers on the day you sign.

    How it works

The health care power of attorney, the living will, the financial power of attorney and the rest, with each state's signing rules, a status for each and the signed copy uploaded. Signing Mode on Communicate gives you nine quiet answers, from Yes and No to I want to talk to the attorney alone and Stop. The answers are yours.

Start with the Care Team page: one card for each doctor.

Your people

You decide who comes in, and what each person sees.

Care Team helps manage your care: the doctors, nurses, therapists and clinics. Home Team helps you at home: the professionals who come to the house. Family and friends join the circle by your invitation.

  • Invite family and friends, each with a role

Each person gets a personal link that works once, with the role you choose.

    How it works

Full access for the one or two people you trust to help run things. Family for the feed, the Love Bucket, photos and updates, never the medical pages. Friends for the friends feed and the Love Bucket. Only you, as the account owner, hand out full access.

    Invite family and friends →
  • Each person sees only their part

The walls are enforced on the server, not only in the menu.

    How it works

Clinical logins see the shared clinical window: the day plan, medications, targets, tracking and notes, never the family feed. Family accounts never see the medical pages, the vault or the bills. Public links, such as a visit link, carry no medical information.

    Who can see what →
  • My People, with a Speak button

Real Call, FaceTime and Text buttons for the people who matter to you.

    How it works

Each person's Speak button says a line like Please call my daughter out loud. Your emergency contacts sit together, and Speak to everyone speaks a line to the whole room.

    My People →
  • Your own key, on your own tablet

A 5 to 10 digit number code on the device you mark as yours: tap the code and you are in.

    How it works

In Settings, under Privacy and Security, set the number code and mark this device as yours, and the sign-in page there shows a number pad. If a family member set the account up, the same place gives you your own email and password on it.

Your own key and device →

Getting help

Help from the people you chose, and a private line of your own.

  • Get Help Now

One tap says I need help now out loud, then gives you five seconds to cancel.

After five seconds the alert goes to the people you gave full access and the professional on shift, by phone notification, text and email. If no one answers within three minutes, your emergency contacts get a text and an automated call. The first person to answer stops the ladder, and your screen says who is on the way. The app never calls 911 by itself.

    Get Help Now alerts →
  • I am not safe

Your own door to outside help, for when the people caring for you are the problem.

Directly under Get Help Now, on your own view of the Emergency page. One tap calls 911, the Eldercare Locator to report neglect or abuse, the 988 Suicide & Crisis Lifeline or the Veterans Crisis Line. Behind a private code only you know, keep one trusted person to tell and a private note. Nothing on the card is spoken out loud, and nothing on it alerts your family, your home team or your care team.

    I am not safe →
  • The Emergency page

Emergency is the last link in the menu, in red, on every screen.

Emergency phrases spoken with one tap, pain and where it hurts, your emergency contacts and your critical medical information in one place. If someone is in immediate danger, call 911 or your local emergency number.

The Emergency page and SOS →

If you are in danger right now, call 911. The app never calls 911 by itself.

One team, one plan

Built for the interdisciplinary care team

An interdisciplinary care team is a group of different health professionals who work together closely with the patient and family to create a single, shared treatment plan. ExpressAgain is the platform where all of those parties care for one patient together: the same day, the same medications, the same notes, and the same record of who did what and when.
