The journal and the printable Care Team Report

One-tap mood check-ins for her, a private journal for each family member, and a chronological printout of what the family recorded.

1 min readUpdated For: patient, family

My Journal is where she taps how she feels — the check-ins speak for her — next to her vitals and the Care Team Report. Each linked family member keeps their own private journal too: the caregiver role sees a Caregiver Journal, family and friends a Family and Friends journal, with moods, reflections and optional notes for the doctors.

The Care Team Report compiles what the family recorded — mood check-ins, observations, care events, vitals — into one clean, dated, printable document to hand to a doctor or a new specialist. It is the family's own records, exactly as written, with who and when; no interpretation, no scoring. Owner and caregiver-role accounts open it.
