View Care Plans: every plan, its versions, the Main plan and Compare plans

View Care Plans, at the top of My Care Plan, lists every care plan for the patient: the family plan and each clinician's own plan, with every version saved, the uploaded plan documents, the Main plan pill and Compare plans.

At the top of My Care Plan (/care-team-plan) is a row of three links, each its own page: My Care Plan, View Care Plans and Compare plans. On a phone they sit two by two; on a wide screen in one row. A clinician on the Care Team finds the same row at the top of the Home Care Team Plan on the care-team dashboard, and an organization's clinical person on the client's plan in the organization panel.

A patient can have more than one care plan. The family plan is the Home Care Team Plan the family keeps, from the family's side. A clinician who evaluates the patient, for example a nurse after an evaluation visit, can start a plan of their own with the same 25 sections: on View Care Plans, under Start your own plan, pick the evaluation date and tap Start from the family plan (a copy of what the family plan says, to change from there) or Start blank. The plan is named after the clinician and carries the clinician's first name, last name, credential, discipline and organization, and the evaluation date. Several clinicians can each keep one. A clinician can start a plan when the family turned on Can fill in the Home Care Team Plan directly on that clinician's card, or, for an organization, with a seat on this client.

What a plan writes is its own: the plan header, the preferences, each section's own lines. A value with a home elsewhere, such as the date of birth, the address on the Emergency Plan Card or a medication on Prescriptions, is the patient's own record and reads the same in every plan; it changes only on the family's pages.

Who sees what. The account owner and caregiver-role accounts see the family plan and every clinician's plan shared with the family; a clinician's plan is shared with the family when it starts, and its clinician can tap Stop sharing with the family. A clinician sees the family plan as before and their own plan, never another clinician's. A family-role account and a friend do not see any care plan, as before.

Save a version keeps the plan exactly as it reads now, every section and line, so it can be read later even after the records it was built from change. On My Care Plan, under the progress, the line reads the latest version, for example Version 3, saved Oct 30, 2026 by Dee Sample (Account owner); tap Save a version, type what changed if you like, and tap Save version 4. The account owner and caregiver-role accounts save versions of the family plan, and so does a clinician or an organization while it fills the plan in directly; a clinician saves versions of their own plan. Once a version is saved, Version and Version date in the plan header read the latest saved version; a version typed there before is kept under the field.

Some versions save by themselves. When the family accepts a clinician's or an organization's proposed sections on Care team, a version of the family plan is saved, stamped with the clinician's name and discipline and the family member who accepted it. When a section of an uploaded plan document is confirmed, a version is saved too. Several of these in a row from the same person within 20 minutes gather into one version, which says how many changes it holds. A version saved by hand is never joined.

View Care Plans lists each plan with its versions, newest first, ten at a time with Newer and Older: the version number, the date, who saved it with their role or discipline, the source (Saved by hand, A clinician's proposal accepted, From the uploaded plan, confirmed, A line copied from another plan, Plan started) and the note. Filter the versions opens a filter by who saved it and by dates. Tap a version to open it on its own page, read only, each section folded to one line: Print prints it whole, and Compare with current lists only what changed since, each line as Then and Now.

The Main plan. One plan carries the Main plan pill. Once a clinician's plan is shared with the family, the most recent one is the Main plan; until then the family plan is. The account owner can tap Make this the Main plan on any plan the family sees; the choice shows who made it and when. Main plan means the plan everyone works from first; it is not a medical order, and no plan here replaces a doctor's orders or a licensed professional's plan of care.

Compare plans puts two plans side by side, the Main plan and the family plan to start; pick others from Plan A and Plan B. Each section is one line with its counts (same, different, only in A, only in B); open it to read every line marked Same, Different, or Only in the plan that has it (missing from the other). Show only what differs is on to start. Where you may change a plan, a line that differs carries Use this line in that plan: the account owner and caregiver-role accounts copy a clinician's line into the family plan, and a clinician copies a family plan line into their own plan. Each copy saves a new version of that plan, source A line copied from another plan. Nothing is ever merged by itself.

Uploaded plan documents. Every care plan document uploaded from My Care Plan is listed under Uploaded plan documents with when and who, the one on the plan now marked On the plan now. An older document stays listed and opens after a newer one replaces it; Remove on My Care Plan takes the current one off. The Day, the daily routine by time of day, is listed with a link; it keeps one live copy and no saved versions.

Closing the account deletes every plan, every version and every uploaded plan document. An organization's reads of the saved plans, its saved versions and its clinicians' plans are in its access log.
