At the top of the Home Care Team Plan is a card, Start from your own plan. If a home care agency, a hospital or anyone else already wrote a care plan or a care team proposal for the patient, the account owner or a caregiver-role account can upload it there: a PDF, a Word file (.docx) or a photo of each page, up to 20 MB. A photo from an iPhone (HEIC) is turned into a JPEG on the phone before it is sent. The file goes straight to the app's private storage, the same place Important papers keeps its copies, and only the family side of the circle and the managing agency can open it.
The app then reads the document and proposes what goes into each of the plan's 25 sections. The card becomes Review the uploaded plan with the number of proposals waiting. Each proposal is shown one at a time with the section's number and name, the proposal in plain words, From the document with the lines it was read from, how confidently it was read, and Open the document to see the original. A section the document says nothing about is simply not proposed; the app never fills in what the document does not say.
You decide on each proposal the same way as Review what we found on Getting started: Looks right keeps the proposal, Change lets you type the section in your own words and Save my words, and Not now leaves the section as it is. Nothing in the patient's records changes until a proposal is confirmed, and every confirmation is stamped with who did it and when.
A confirmed Weekly Coverage Plan adds the document's shift pattern to the weekly schedule on Home Team management as open slots, nobody yet, so you pick who holds each one there. A confirmed Daily Routine adds the document's blocks to the patient's Day, skipping a block the Day already has. Every other section keeps the confirmed words on the plan itself: open the section lower on the page and it shows them under From the uploaded plan (confirmed by your name), with a source line that opens the document.
Things in the document that are not plan sections, such as an hourly rate, the weekly hours, the weekly and monthly totals, the billing cycle and the signature block, are shown in one box, Also in this document, each marked In the document. The box offers the two places those things live, My Home Team for the rate and Home Team agreements for the agreement, and nothing is written there until you set it on that page yourself.
One upload at a time: uploading a new document replaces the proposals still waiting and never touches a section already confirmed or changed. Replace swaps the document; Remove takes the document and the waiting proposals away and keeps the confirmed sections.
An agency that manages a client's account (an account the agency created) sees the same card on the client's Manage view, under Home Care Team Plan, from its office or administrator login. While the account is not yet claimed, the agency confirms proposals itself and each one carries the agency's name. After the family claims the account, the button reads Looks right, send to the family, and each confirmation goes to the family as a proposal to accept or decline on the Care Team page, under what the agency manages.
Family-role accounts, friends, clinicians and Home Team professionals do not see the card. If reading a document is not set up on a server, the card says so and the file is still kept.
