Agreements on real paper: fill, send, sign, and the certificate

The founder's agreements as real letter-size pages: the Caregiver Services Agreement (the South Carolina version for a home in South Carolina) with the Home Access and Property Security Agreement, signed by you and the professional, with a certificate of completion as the last page.

On Home Team agreements, New agreement starts the Caregiver Services Agreement, the agreement for a family that hires its own professional directly. It covers four documents signed together, once: the agreement, its Schedule 1 (household care instructions), the Home Access and Property Security Agreement (keys, codes, cameras and property) and its Schedule 1 (a property inventory you may leave out). When the home is in South Carolina, the South Carolina version is used; the state comes from the home's address on file and you can change it on the form. Other agreements offers the Marketplace agreements: Agreement 2 for a professional without a license, Agreement 3 for a licensed or certified professional and Agreement 4 for a licensed agency, each with Addenda A, D and E, and the South Carolina rider (Addendum C) when the home is in South Carolina.

The words are the founder's, exactly as written; the form changes only the blanks. Your name, the professional's name, the patient and the home's address come from your records; you can correct an address. Every other blank (the start date, the schedule, the rate, the boxes for the duties) is yours to fill; a blank you leave empty prints as a line to fill by hand. The paper below the form is exactly what will be sent.

Send for signature freezes the text: the day you send it becomes the Effective Date, and a SHA-256 fingerprint of the exact text is kept. The professional reads it on paper in their dashboard, types their full name, ticks that they have read it and agrees to do business electronically, and signs; then you sign for the family the same way, with your relationship to the patient. ExpressAgain is not a party to these agreements and does not sign them.

The signed copy looks like a real signed agreement: each signature block has the name in handwriting, the printed name, the relationship or title, the date and Signed electronically with the time. The last page is the Certificate of Completion: the document number, the version, the SHA-256 of the frozen text, and for each signer the name, role, email, the time with its time zone, the network address, the device, the consent they accepted, and the events (sent, viewed, signed, countersigned). Print or save as PDF opens the same pages, one letter sheet per page with Page 2 of 9 and the document number in each footer.

Step by step

  1. 1Open Home Team agreements and tap New agreement (or Other agreements for a Marketplace hire).
  2. 2Pick the professional, check the state the home is in, and switch optional documents on or off.
  3. 3Fill the blanks; read it on paper below the form.
  4. 4Tap Send for signature. When the professional has signed, open it and tap Sign for the family.
  5. 5Tap Print or save as PDF to keep the signed copy with its certificate.
