Client intake: the in-home intake, your own e-sign, start of care and the counts

Do the in-home intake on a tablet one section at a time, have your own consents and service agreement signed on the tablet or by a private link, start care by creating the client without sending anything, then invite the family when you confirm; Sources and conversion counts where families came from and how many started.

The in-home intake. On the family's In-home intake page, the checklist bar shows eight sections and how many are done; each opens its own page, made for a tablet in the family's home: the home safety walk-through (entry and steps, rugs, grab bars, a shower chair, lighting, smoke detectors, pets, smoking, weapons stored, an emergency exit, and notes), activities of daily living (bathing, dressing, toileting, transferring, continence and eating, each Independent, Needs some help or Needs full help, plus the household tasks), the mobility level (the same transfer scale as the screening, which it starts from, the number of people, falls in the last six months, the equipment), medications (each one with its dose and when, who manages them, and your medication rule from your agency profile), emergency contacts with first and last names, the doctor and the pharmacy, the care plan draft (the tasks, the days and hours, the start date, the goals in the family's words), consents and the service agreement, and payer forms (the payer and the policy or authorization number; private pay uses your own payment link, and ExpressAgain stores no card numbers). Save keeps a section; Save and mark done ticks it on the bar. Back and Next section move one at a time.

Health sections and the Intake permission. Activities of daily living, mobility, medications, emergency contacts and the care plan draft are health answers: they are sealed at rest and read and saved only by people your organization admin gives the Intake permission. Anyone else sees the checklist bar and which sections are done, with a lock line in place of the answers; the home safety walk-through, consents and payer forms are open to the office.

Your own consents and service agreement, signed. Your administrators upload your own PDFs under Intake settings, Packet files, as a consent form or a service agreement. In Consents and service agreement, each one says Not signed, Sent for signature, or Signed with the day and the signer. Sign on this tablet asks for the signer's first name, last name and what they sign as (the client, power of attorney for the client, or the family member responsible for payment), then the same signing panel ExpressAgain uses for its own agreements: the name as it will appear, I have read and agree, and the consent to electronic records. Send for signature instead prepares a private link for a family member who has no account, which goes only after the preview and your confirm; it opens the PDF on the signing page, works once, and lasts 30 days. Every signed copy keeps the time, the network address and device, the consent's exact words and a fingerprint of the exact PDF. Only your agency's own consent and agreement PDFs can be sent this way; ExpressAgain does not draft your contract.

Start of care. Four steps, and nothing goes to the family until the fourth: Create the client uses the same Add a client form as Patients, filled in from the inquiry, without the family's email or cell, so saving sends nothing; the account belongs to the patient's family, never the agency. The first schedule adds the care plan draft's days and hours to the weekly schedule through the usual schedule change (a home that has not delegated its schedule gets it as a proposal to approve). Match a professional lists your own professionals with what fits the needs found (Transfers and lifts validated on a skills checklist, heavy transfers, two-person transfers, mechanical lift trained), for people with the Intake permission; Pick only notes the choice on the timeline, and nothing is offered to anyone. Invite the family sends the claim link with Send the family their link, after a confirm. What the assessment fills carries the mobility level onto the patient record, the contacts, doctor and pharmacy onto the Emergency card, and the medications onto the list, each through its usual page and its own rules, as a draft your clinical person reviews. Then Mark care started.

Sources and conversion. Under Reports, Client intake: sources and conversion (or the Sources and conversion tab in Client intake) shows, for the last 90 days, this month, this year or everything: the inquiries by source, how many were screened, assessed and started care, and the share that started; the same by referring organization; and why families did not start. These are counts only: no referral fee is ever recorded or paid on a patient referral, and no health answer is in the report.

Intake settings in one place. Intake settings holds Clients we accept (the transfer answers start from your Marketplace profile until you save here; dementia, wandering, physical aggression, smoking, pets, the shortest visit, the payers you accept and the ZIP codes you serve), your own screening questions after the standard ones, the lost reasons and where you refer families you can't serve, what to have ready, the message wording with its fill-ins, your packet files, and the Intake permission.
