Send the home summary to the doctor

The doctor's home summary opens with what changed since the last visit and the readings outside the family's own targets; Send to the doctor saves it as a PDF and opens the patient portal, where you attach it and send it, so it lands in the chart.

The home summary for the doctor is on Doctor's Records (Send to the doctor) and opens at /records/summary. Choose the days at the top: Since the last visit (from the last doctor's visit on the schedule, up to 90 days back), 7 days or 30 days. With no earlier visit on the schedule, Since the last visit covers the last 7 days and says so.

It opens with What changed since the last visit. Medications: the doctor's list and the home list puts the medication list in the connected chart next to the list the family keeps in the app, side by side, each as its own list words it, because the family often updates a medication before the chart does. Each difference is stated as a fact: Strength differs (both numbers), Status differs (for example stopped at home, active on the doctor's list, or on hold in the chart), Only on the doctor's list, Only on the home list; the medications both lists agree on are named in one line. With no chart connected there is no doctor's list to compare, and the page says so. Then the medication changes recorded at home in the period (the dose history, and medications added), the symptoms logged (how many times, first and last day, and whether it is the first time in the 90 days before), falls, ER visits and hospital stays (falls and help-button incidents from the home log, stays and ER visits from the chart), and the appointments in the period with what became of each, and the next one booked.

Next comes Outside the targets the family entered: for glucose, blood pressure and weight, how many readings were outside the targets the family typed in the app, and on which days, for example BP above the family's target of 140/90 on 3 of 14 readings. Nothing is counted without a target, and the line says so. The summary never calls a reading abnormal and never gives advice: it records what was entered. The week's detail follows, as before: medications and every dose, glucose, blood pressure, weight, symptoms and events, meals, home-care visits and notes for the doctor.

Send to the doctor: the patient portals let apps read the chart but not post to it, so ExpressAgain cannot put the summary in the chart for you. One tap does the rest of the work. On a phone, the share sheet opens with the PDF file, to save it to Files; on a computer, the PDF downloads and the connected place's patient portal opens in a new tab. Then: attach the PDF you just saved, choose the doctor, and send. Messages sent through the portal are saved in the chart. For MyChart, Epic's lists carry no organization's MyChart address, so Open MyChart opens Epic's MyChart site, where you sign in or find the organization; healow opens healow's sign-in, and VA opens VA.gov's secure messages. Oracle Health and athenahealth have no single patient portal for every hospital or practice: the account owner or a caregiver-role account can save the place's own portal address once, in the Send to the doctor panel, and so can save an organization's own MyChart address.

After sending, tap I sent it (or I printed it or handed it over) with who you sent it to. The summary page then shows Sent to Dr. Physician via MyChart, Oct 24 · noted by the person who tapped it. It is your note of what you did, not a receipt: ExpressAgain cannot see the portal and never says the summary was delivered. Print / Download PDF prints the same report. The summary is never emailed in plain text.

The reminder: one or two days before each doctor's visit on the schedule (from the chart, or a visit typed as a doctor, specialist or telehealth visit), whoever runs the account and the caregiver-role accounts get a phone notification, Before the doctor's visit, and the appointment on Appointments reads, for example, Dr. Physician's visit is Thursday — send the home summary? with Send to the doctor. Once a send is noted for that visit, the line goes away. To turn the reminder off, open Settings: Family & Friends, Doctor visits, for the account owner, or Notifications & Alerts for a caregiver-role account, and switch off Remind me to send the home summary before a doctor's visit. The phone notification also follows the Appointments switch under phone notifications.

Who: the family's accounts send and note it; a clinician on the care team can open and print the summary but does not send it from here. The summary, the reminder and the switch appear once Doctor's Records is switched on for the deployment.

  4. 4In the portal, start a message to the care team: attach the PDF you just saved, choose the doctor, and send.
  5. 5Back on the summary, type who you sent it to and tap I sent it.
