One care plan per patient. Whoever starts it, the family owns it.
Whoever starts the plan, it is the same plan: the family claims the account by a private link and owns it, and everyone who helps works from it as their role allows. Nothing is typed twice.
Only the first name here; the rest is typed once, where the plan lives.
