The patient's own voice in the record: shift reviews, care checks, Report a concern

After each shift the patient's own login says how it went, confirms whether a logged turn happened, and can report a concern in the patient's own words; the family reads it all on Home Team reports, the professional never does.

These cards show only on the patient's own login: the patient key on the shared account (set under Settings as the second email and password or the number code) or the owner of an account the patient signed up for. A family login sees nothing of them. They sit on the Dashboard and the Day page, under the welcome card. Every answer adds to the family's record; nothing here is a way to dismiss anything, and the professional on shift never sees a review, an answer or a concern.

How was this shift? appears after a professional clocks out, for 24 hours. Three big buttons: I was looked after, I waited too long, I asked and nobody came. After a tap the card says it is saved, offers Add a voice note (recorded in the browser and kept with the family's private media) and, when the professional works through an agency, Also tell the agency. One review per shift; it can be changed for 24 hours, then it stands. I asked and nobody came reaches the account owner at once, by email and phone notification; every review goes to the other caregiver-role accounts in the day's digest email.

Were you turned at 2:00 PM? appears when a professional ticks an expected care event under During the shift on the Checklist tab, or logs Turned / repositioned (or another personal-care step) in the personal care log. Yes and No are the only buttons. One answer per event, within 24 hours of it. A No reaches the account owner at once and shows beside the event on Home Team reports; the professional never sees the answer.

Report a concern takes the patient's words, typed or spoken, with the time and who was clocked in right then. It goes to the account owner at once and to the other caregiver-role accounts in the day's digest, and to the agency office only when Also tell the agency is ticked. Never to the professional on shift.

On Home Team reports, The patient's own word lists every shift in the range with the professional, the checklist line, the expected care recorded or not recorded, the shift review with its voice note, and the care checks answered No, with the concerns under the table; Calls for help lists each Get Help Now press with how long until someone said they were on it, by shift and by professional, together with the worn help button's presses (the record keeps each press but not the time someone came, so those count without a wait). Both print and download as CSV. The Care Team Report under My Journal carries the same Calls for help block for its range. On Home Team management, This week, each shift row carries the review and the gaps beside its checklist chip.

The agency office sees a review or a concern on its Team tab only when the patient ticked Also tell the agency on that item, and only about its own staff. It never sees a care check or hears a voice note.

Step by step

  1. 1Sign in with the patient's own key; on the Dashboard, under How was this shift?, tap I was looked after, I waited too long or I asked and nobody came.
  2. 2Add a voice note or tick Also tell the agency if you want; tap Change to pick another answer within 24 hours.
  3. 3When a card asks Were you turned at 2:00 PM?, tap Yes or No.
  4. 4Tap Report a concern, type or say it, then Send to the family.
  5. 5As the family, open Home Team reports, then The patient's own word or Calls for help; tap Download CSV to keep a copy.
