Expected care events: a turn every 2 hours, a skin check at the start and the end

Checklist items that repeat during every shift, ticked one occurrence at a time, so a missed turn is a named gap on the shift instead of a blank.

3 min readUpdated For: family, professional

Under Home Team management, Shift checklists, the section Expected care, every shift holds the care that repeats during a shift. Each item has a repeat rule: every N hours during the shift (half-hour steps, up to 12), at set times on the home's clock, or at the start and the end of the shift, with a window either side in minutes. Suggestions sit ready to add: Turn and reposition every 2 h, Skin check at the start and the end, Offer water every hour, Toileting at set times. Tap Save expected care. These items never gate pay release and are not on the start or end list themselves.

On the professional's Checklist tab, During the shift lists each item with its occurrences worked out from the clock-in: Turn and reposition at 9:00 AM, 11:00 AM, 1:00 PM and so on. The professional taps each one as it happens and the app records the time. An occurrence whose window closes without a tap reads Not recorded; it can still be tapped if the care was done. A missed occurrence is also listed on the End of shift card as Not recorded, by name.

The family sees the same on Home Team management, This week (a chip on each shift row, for example Turn and reposition 3/4, opening to the named gaps) and on Home Team reports, The patient's own word, shift by shift, with CSV. Section 10 of the Home Care Team Plan, Wound and Skin, shows the turn schedule, the skin-check rule, the cushion and mattress on file and the last skin check from the personal care log, with a missing line for whatever is not set.

An agency that manages a home through ExpressAgain can add an expected care event in its own Manage page under Shift checklist, When: During the shift (repeats); on a home that has not delegated the plan it goes to the family as a proposal to accept. Where the family and the agency set the same item in the same words, the family's version is the one used.

When a professional ticks an occurrence, the patient's own login may be asked whether it happened; see The patient's own voice in the record.
