Log it is the green microphone in the app header, next to the blue Listen button, for the professionals who sign in to the care-team dashboard (an aide, a CNA, a nurse, a clinician on a seat for this patient; the agency office does not see it) and for the family's account owner and caregiver-role accounts. Tap it, say one sentence about what just happened, and a card lists what will be logged, in plain words, with the time. Tap Log it and each line goes on the same log a tap on Health Tracking, Prescriptions or the kitchen would have written, stamped with your name and the words by voice. Under the microphone is a box to type the same sentence; the keyboard's return key sends it, so everything works without a microphone.
What you can say. Bathroom: she peed, she had a BM at 3:45 pm, she wet the bed an hour ago, BM in the brief (an accident or a brief is kept on the entry). Meals: I cooked dinner, she had chicken and rice, ate most; breakfast oatmeal, she ate half; lunch was a sandwich, she ate a little. The foods go on the meal, with how much was eaten, and any food that matches a kitchen row by name is marked used in the kitchen the way the Meals tab does it, so the kitchen count comes down and the professional's use shows under What professionals used; a food that fits two rows equally is left alone, never guessed. Readings: glucose 100 before dinner (or after breakfast, fasting, bedtime; with no column said the card picks one by the clock and offers the others), BP 130 over 80, pulse 72, temperature 99.4, oxygen 96 percent, respirations 18, pain is a 6 out of 10, weight 162. Symptoms, by the catalog's own words: she's crying, she was angry, she got aggressive and was yelling, UTI test positive (or negative, or waiting), her right leg hurts, pain in her chest, she fell at 11, her feet are swollen, she's nauseous, very lethargic, she couldn't sleep, she pressed the help button. Sleep: she's taking a nap, she woke up from her nap at 3:15, she went to bed at 9:30 pm, she's up for the day. Personal care: gave her a shower and she was steady, legs gave out in the shower, got her dressed and needed help, turned her at 2, brushed her teeth. Water: she drank 8 oz of water. Several sentences in one breath become several lines on the card.
Times. Say at 3:45 pm and the entry is 3:45 PM; say at 3:45 and it is the latest 3:45 that has already passed; an hour ago, 20 minutes ago, this morning, last night and yesterday at 10 am all work; say nothing about the time and the entry is now. Every time is read on the patient's clock (the circle's time zone in Settings). A time later than now today is never written: the card asks whether you meant yesterday or now. On the card, Edit time opens a time field for any line.
Doses. The founder wants medications by voice too, so gave her Lantus 15 units, she took her morning meds, she refused her Sample Brand A, skipped the Lantus tonight and we're out of Lantus all work, naming the medication from the Prescriptions list (the 6 pm Lantus picks the 6 PM dose; morning meds proposes one line per scheduled medication with a morning dose). Every dose line carries its own Confirm button and nothing is logged until it is tapped, by hand, every time, hands-free or not. A dose goes on the dose log exactly as a tap on Prescriptions would: given, skipped, refused, or out of medication, which uses the same out-of-medication path that tells the family and the care team.
The card. Each line reads like Bathroom · urinated · 3:45 PM, Dinner · chicken, rice · ate most · 6:10 PM · kitchen: Chicken breast, Rice marked used, or Glucose 100 · before dinner · now. When the app was not sure it asks one question with chips on that line (Pee or BM?, Which leg?, Which meal?, Which column?, Which medication?, Result?) and the Log it button waits until it is answered. A line marked Check was a best guess; look at it. Remove drops a line. A sentence the app cannot place becomes a note on the IDC Team feed, in your words. When an AI key is set for the app, a sentence the plain rules could not read is sent once, as text only, to be sorted into the same shapes with the catalog's names and the medication list as the only words it may use; it only proposes, it never writes, and without the key the sentence simply becomes a note.
After Log it the card says Logged and reads it aloud in the app's voice, and each line shows Undo for 30 seconds. Undo removes that entry through the same delete that the log's own X uses; a meal taken back puts its food back in the kitchen. After 30 seconds, fix an entry on its own log as always.
Hands-free. The switch Hands-free while the app is open: say Hey Owl is in the Log it panel and is remembered on that device. While it is on and this page is on the screen, the phone listens for the wake word (Hey Owl until the owl has its name; the word is one setting in the app). Say Hey Owl, she's peeing right now, log it, or Hey Owl and then the sentence, with earphones in and the phone nearby: the card opens, reads the lines back, and logs them after five seconds unless you say no or tap No. A card that needs an answer, or a dose waiting for Confirm, waits for your tap instead. Honest limits: listening runs only while the app is on the screen. On iPhone, Safari stops listening when the screen locks, when you switch apps, and after about a minute of silence; the app starts listening again by itself when you come back to it, and the switch stays on. Android Chrome behaves much the same. Nothing is recorded: the words are turned into text on the phone by its own speech recognition, and a sentence without the wake word is dropped on the spot.
From the home screen or Siri. The app's home-screen icon has a long-press shortcut named Log it that opens the app with the microphone armed; so does the care-team app's icon. To say it to Siri instead: open the Shortcuts app, tap the Add button, tap Add Action, choose Open URLs (under Web), type https://www.expressagain.com/home?logit=1 (a professional types https://www.expressagain.com/idc/dashboard?logit=1), tap Done and name the shortcut Log it. From then on, Hey Siri, Log it opens ExpressAgain with the microphone ready, and you say the sentence. On Android, the Google Home app's Routines can open the same address when you say a phrase of your choice. No audio ever reaches ExpressAgain from Siri or the Assistant; they only open the page.
Step by step
- 1Tap the green Log it microphone in the app header (or type in the box under it).
- 2Say one sentence, for example she peed at 3:45 pm, glucose 100 before dinner, or I cooked dinner, she had chicken and rice, ate most.
- 3Read the card: answer any question chip, tap Confirm on a dose, use Edit time or Remove if a line is off.
- 4Tap Log it. The card says Logged and reads it back; tap Undo on a line within 30 seconds to take it back.
- 5For hands-free, turn on Hands-free while the app is open and say Hey Owl, then the sentence; say no within five seconds to stop it.
