Partners: hospice, equipment and others

A hospice liaison, a wheelchair supplier's ATP, a pharmacy: people who work with the family but are not the care team or the Home team. They have their own Partners section on Care Team, a small partner page of their own that never shows the patient's record, an optional weekly summary the family switches on, reach-out reminders that alert the family, never the partner, referral tracking (who referred, when, what for, and where it stands), and one login for a partner organization that shows only the families who allowed its team.

Some people who matter to the patient's care are neither the care team (the doctors, nurses and therapists who help manage the patient's care) nor the Home team (the professionals who help at home). A hospice clinical account executive or a hospice liaison may become the patient's hospice later; their job is timing. A complex rehab or wheelchair supplier, with a licensed ATP, often comes in through the occupational therapist and stays for the years of the chair's warranty. A pharmacy fills the prescriptions. On ExpressAgain they are Partners: one kind, with a type on each card: Hospice & palliative, Equipment & wheelchairs, Pharmacy, or Other.

Where they are: Care Team, on the first tab, in their own Partners section below Doctors, nurses & clinics. A partner is never in the doctors' cards, never on the Home team tab, never on the Home Care Team pages, the IDC Team feed or the care-team dashboard. Only whoever runs the account and caregivers see and manage partners.

The card: the name, the organization, the type, a title, phone and email, Brought in by (the person on the care team who brought them in, picked from your Care Team cards, optional; the card then reads Brought in by Sample OT, Occupational therapist), Why (one line), Relationship until (an optional day, for example the warranty's end) and notes. The pen edits the card in place. Open goes to the partner's own page on Care Team: Access, Updates, Visits, Reach-out reminders, Messages, Shared documents and the Activity log, where every change, switch, reminder, share, message and sign-in is listed with who and when.

What a partner sees with a login: Give access on the partner's page makes a partner code (and emails it when you tick Email it); the link opens the IDC Team sign-in, which reads Joins the patient's care as a partner, and the partner chooses a password. Their partner page shows the patient's first name, their own visits with the patient (Book a visit puts one on the calendar where the time is free, the same no-overlap rule as the booking link; the calendar reads Partner visit with the time, so the rest of the team sees the time is taken, never who), one message thread with the family (whoever runs the account and caregivers), and the documents you share with that partner, one at a time. Nothing else.

What a partner never sees: the medication list, the daily logs and Health Tracking, the care-team and family feeds, the care plan, or anyone else's contact details. This is enforced on the server: the partner's sign-in opens only the partner page's own routes, and every other part of the app answers as if nobody were signed in. End access stops the sign-in at once; Remove the partner also takes back every document you shared.

Shared documents: on the partner's page, pick one of your Important papers that has a stored copy, or upload a file just for that partner (a PDF or a photo). Stop sharing takes it back at once.

Updates, off until you turn them on: Send them the weekly summary, on the partner's page, shares the same weekly summary the doctors get (This week for the doctor: readings, medications and doses, events and meals for the last seven days) behind the partner's sign-in, with an email each Monday morning that carries no health details. You can switch it off any time, and every change is in the Activity log. It needs the partner to have a login, and it follows the site's own summary setting: where This week for the doctor is not switched on, the partner's link says it isn't set up yet.

Reach-out reminders alert the family, never the partner. On the partner's page, Reach-out reminders offers ready-made rules for the type. Hospice & palliative: an ER visit or a hospital stay logged (Care Team, Log a call or a visit), falls logged within a few days (2 falls in 7 days to start), a weight logged a number of pounds you choose under the highest weight logged in the 30 days before, and a medication for pain added to the list (its Reason says pain). Equipment & wheelchairs: a fall logged, a skin or pressure area logged (Health Tracking, Symptoms & events, Skin or pressure area), and a PT or OT note tagged Equipment no longer fits. Every rule is available on every partner under More rules. Only what is logged after you set a rule counts, and each thing logged reminds you once.

When a rule finds something, you get Time to reach out? on your phone (no names, nothing that was logged; the Partners switch) and a reminder at the top of Partners: what was logged, in plain words, the question (Message Brandon?, or Message the OT and Brandon? when someone brought the partner in) and a suggested message you can change. Send puts it in the partner's thread (when they have a login); Text and Email open your own phone or mail with the words in it, then I sent it myself records it; Dismiss closes it. Nothing is sent until you tap Send. The rules count what you or the team logged; they never score, judge or predict anything.

Referrals, tracked: when a physician, an OT, a case manager or a social worker refers the family to a partner (the OT referred the wheelchair supplier for a power wheelchair evaluation; the doctor referred a hospice for a hospice evaluation), the partner's page has a Referral section, first under the card. Record a referral: who referred (pick one of your Care Team cards, or Someone else and type the name and role, for example Sample OT, occupational therapist), the date referred, what it's for (for example Power wheelchair evaluation) and the reason, which only the family sees. Where it stands now starts at Referred; pick another status when you are recording an older referral.

The status moves Referred, Contacted, Evaluation scheduled, In progress, then Done, or Not going ahead. Update the status, with an optional note (for example Thursday at 10, at home), and every change goes in the referral's History with who and when, and in the partner's Activity log. Edit details changes who referred, the day, what it's for or the reason. The Partners card shows the open referral as a chip, for example Referral: Evaluation scheduled. A partner can have more than one referral over the years (a new chair after five years): one is open at a time, and the earlier ones are listed under Earlier referrals, newest first. Record a referral shows again once the open one is Done or Not going ahead.

Tracking only: ExpressAgain records that a referral happened and where it stands. Nothing is bought, paid or filed through it, and nobody is paid or rewarded for a referral.

What the partner sees of a referral, with a partner login: a Referral tab on their partner page with what it is for, who referred (the name and role exactly as you entered them, never a phone number or an email), the date, the status and its history. The partner can move it forward (Contacted, Evaluation scheduled, In progress or Done) with a note for the family; the change is logged as by the partner and you get A partner updated a referral on your phone. A partner never sees the reason you wrote, never sets Not going ahead, and never sees anyone else on the care team.

A gentle check-in: when a referral is still Referred or Contacted 14 days after its last status change, you get Time to check in? on your phone (no names) and a reminder at the top of Partners, for example No update on the power wheelchair evaluation in 2 weeks: still Contacted since Oct 8, with Check in with Brandon? and a suggested message you can change. As with every reminder, nothing reaches the partner until you tap Send (or text or email from your own phone). A status change closes the reminder; each quiet stretch reminds you once.

One login for a partner organization: a hospice or a supplier whose people work with several families can have one organization login that shows the families who let its team see a partner card, and only those. On their partner page, a partner opens Your organization's team, types the organization's name and type and their partner password, and taps Make the organization; they become its admin. It is the organization's own small record on ExpressAgain, apart from any family's account.

Each family decides. A partner connects their own cards (Connect your partner cards, with that partner login's email and password); each family gets a request at the top of Partners, for example Brandon wants to connect Sample's card to Sample Medical. Allow Sample Medical's team to see it?, with Allow and Not now, and a phone notification with no names. Nothing changes until the family taps Allow. On the partner's page, Team has the switch Let Sample Medical's team see this too: off until you turn it on, every change in the Activity log, and turning it off ends the team's access at once. After Not now, only the family can turn it on. Ending the partner's access or removing the partner turns it off too, and it stays off until you turn it on again.

What the team sees: the organization's list shows each family that allowed it, by the patient's first name only. Open shows the same partner page the named partner sees: the visits, the one message thread, the documents you share with that partner, the referral, and the weekly summary only if it is on for that partner. Everything a team member does there is logged with their own name and the organization's, for example Sample Colleague (Sample Medical). A family that has not allowed the team never appears, and asking for it by its address is refused.

The team: the admin invites colleagues by email with a one-time code (it works once, for 14 days; they join on the partner sign-in under Your organization, then Have a team code), can make another admin, and removes a colleague, whose login stops working at once. There is no password reset email: a forgotten password needs a new code from the admin (New code), which sets a new password and signs out the older sessions. The organization's people are never care-team logins and never see anything else of the record.

No referral fees, ever: nobody is paid or rewarded for bringing a partner in, and a partner pays nothing to be on the patient's Care Team.

Step by step

  1. 1Open Care Team. On the first tab, scroll below Doctors, nurses & clinics to Partners and tap Add a partner.
  2. 2Pick the type, type the name, the organization, the title and how to reach them, and, if the OT or a doctor brought them in, pick that card under Brought in by. Tap Add the partner.
  3. 3On the card, tap Open. Tap Give access to send their partner code.
  4. 4Under Reach-out reminders, tap Remind me on the rules you want.
  5. 5Under Referral, tap Record a referral: who referred, the date, what it's for and the reason. Tap Record the referral, then Save the status as it moves.
  6. 6Turn on Send them the weekly summary only if you want them to have it; switch it off any time.
  7. 7When a partner asks to connect their card to their organization, tap Allow on the request at the top of Partners, or Not now; later, use Let the organization's team see this too under Team on the partner's page.
  8. 8Working with several families? As the partner, on your partner page, open Your organization's team and tap Make the organization; then, on the organization's page, Connect your partner cards and Invite a colleague.
