The video visit. An appointment of type Telehealth is a video visit. The family books it under Appointments (ID Care Plan, Book an appointment, Type of visit: Telehealth); a clinician books one from the care-team dashboard under Today, Today's schedule, Book my visit, with Video visit (telehealth) ticked. The same row is on the patient's Schedule and on the care-team dashboard the moment it saves. A clinician who is a contact card only (a doctor with a name and a phone number, no login) has no button; the family joins for the patient from Appointments.
Joining. From 15 minutes before the start until the end of the visit (the end time, or an hour after the start when none is set, on the patient's clock), the appointment card under Appointments, the row on the patient's Schedule and the row on Today's schedule carry one button, Join video visit. Before that the card says when the room opens; after, nothing. The button opens the video visit page in the app, which asks you to allow the camera and microphone and then runs the call on Jitsi Meet inside the page, in the browser, with nothing to install and no account, with recording and streaming off; nothing is recorded. The room's name is a random code made on the first join, never shown and never built from anything typed. Leave ends the call. The day-before reminder email for the appointment says it is a video visit and where the button is; no new reminder is sent.
On the care-team dashboard. When a video visit is booked for today, one line above the clock reads, for example, Video visit at 2:30 PM with the patient's first name; a tap opens Today's schedule. The top of the dashboard stays as it is; nothing new is added to the page.
The visit note. After (or during) a video visit or a home visit, a skilled clinician on the care team (a nurse, a physician, a therapist, hospice or palliative care, the community paramedic, the chiropractor; never the Home Team aide or the agency office) writes the note from the visit's row on Today's schedule: tap Visit note under the row. The note is short and structured: Reason for the visit, Findings, Vitals taken (closed until opened: blood pressure, pulse, respiratory rate, SpO2, temperature, blood sugar, weight and pain, each checked against what a person can measure), Medications reviewed (yes or no and a line), Plan, Follow-up (a day and who) and Share with the family, on by default. Save as draft keeps it yours to change; Sign the note makes it final. A signed note is never edited: Add an addendum appends a stamped paragraph under it. When a note is signed, every vital in it also goes to the patient's Tracking as a reading by you.
Who reads it. Every active care-team login for the patient reads every signed note, on the Clinical door's Notes tab under Visit notes, newest first; a draft is read only by the clinician who wrote it. The family (whoever runs the account, the caregiver role and the family role) reads a signed note only when Share with the family is on: it sits inside the appointment's card under Appointments as Visit note from the clinician's name and role, closed until opened, and one line lands on the care-team feed with the plan behind a tap. The agency office never reads visit notes. Nothing in a visit note is emailed or texted.
Step by step
- 1Family: under Appointments, tap Book an appointment, choose Telehealth as the type, pick the day and time, then Book it.
- 2Clinician: on the care-team dashboard, open Today, Today's schedule, tap Book my visit, tick Video visit (telehealth), pick the day and time, then Reserve this time.
- 3From 15 minutes before the start, tap Join video visit on the appointment card (family) or on the row under Today's schedule (clinician); allow the camera and microphone when the browser asks.
- 4After the visit, under Today's schedule, tap Visit note under the row, fill in the reason, findings, vitals, medications reviewed, plan and follow-up, keep Share with the family on, then tap Sign the note (or Save as draft to finish later).
- 5To add to a signed note later, open it under Clinical, Notes, Visit notes and tap Add an addendum.
- 6Family: open the appointment's card under Appointments and tap Visit note from the clinician's name to read it.
