The care chapter: when care moves to hospice, and when the patient has passed

The care chapter page holds where the patient's care is now: everyday care, hospice care with its details, or that the patient has passed. Changing it is one switch behind a confirmation that says in plain words what changes and what does not, and the app quiets the reminders that no longer fit.

Where it is. Open Settings and tap the patient's care chapter (for example Debbie's care chapter), or open the Patient Profile and tap it beside Diagnosis mode. Every member of the circle can read the page. The family's own sign-in and an account with the caregiver role (full access) can change it. The patient's own sign-in shows the one line about where things are now, and never the switch or the links to the planning pages.

Now. The top of the page says where things are, in one line: everyday care, in hospice care since a day, or passed away on a day. In hospice care it also shows the hospice, where hospice care is, the level of care, the hospice nurse and the number to call at any hour (tap it to call), and a short list called Quiet now.

Moving to hospice care. Tap the button that says the patient is now in hospice care. Enter the day hospice care started; everything else is optional: the hospice, where hospice care is (Choose one: at home, at a family member's home, a hospice house or inpatient unit, a nursing home, assisted living, a hospital, or Other with your own words), the name of the place when it has one, the level of hospice care (Choose one: visits at home, around-the-clock care at home for a crisis, a few nights' stay so the family can rest, or inpatient care for symptoms that can't be managed at home), the hospice nurse's first and last name, and the number to call at any hour. A box, ticked to start, opens the phrase library with the Hospice & Palliative phrases first. Before anything is saved, the page lists what changes and what does not, and only the Yes button saves it.

What goes quiet in hospice care. The alerts for readings outside the targets (blood pressure, glucose, temperature, oxygen, pulse, breathing and weight) are no longer flagged alerts: readings can still be logged and the care team still sees them. Pain and every other symptom still alert as before. Medication reminders, the bowel watch, out of medication, drink-water reminders and Get Help Now stay as they are. Change the hospice details saves new details later and posts nothing.

When the patient has passed. Tap the button that says the patient has passed, enter the day, and, if you like, the time in your own words (for example early this morning, at home). These stop: every reading and symptom alert to the care team, medication reminders and the notice when a set is not logged yet, out of medication and running low, the bowel watch, drink-water reminders, the daily reminders on the care team's standing instructions, How was your visit? to the professionals after a visit, the reminder to send the home summary before a doctor's visit, and the circle's daily messages (good morning, lunch, goodnight) and the condition fact email. Nothing is deleted: the record, the photos, the feed and the Love Bucket stay. If an agency or practice that follows the patient keeps a care record in ExpressAgain and it does not say so yet, it is marked with the day of passing. If this was a mistake, the page offers Undo, which brings everything back, takes that mark off and posts a short correction.

Who is told. Each change of chapter makes one post on the family feed, from the app (Care chapter, shared by the family), in plain words: for hospice care the day it started and the details you added; for a passing the day, and that the family will share more when ready. One note goes on the care-team feed, from the person who made the change, so the professionals know. Everyone on the family feed (not friends) and the active care team get one phone notification that says only News from the family; a lock screen never shows the chapter or the name. The person who made the change is not notified.

On the dashboard. In hospice care the family dashboard shows one line, for example Debbie is in hospice care, that opens the care chapter. After a passing it shows Remembering Debbie, which opens the memorial once the family turns one on, and the care chapter until then. In everyday care nothing shows.

The chapter's other pages. Under the switch, a short list links to the chapter's own pages: The weeks ahead, The first days, The weeks after, The service and The memorial. Each is its own page with its own article.
