When a doctor's practice asks to follow her care

A practice enrolls the patient; the owner or a caregiver-role account accepts a link that names the practice and the primary doctor; the family can end it any time.

A physician's practice that uses ExpressAgain can enroll a patient. The family gets an email or a text that names the organization and the practice only — never the patient, never a reading — with a link. The page behind it says who wants to follow her care, who the primary doctor will be, and what they will see. Accept or Not now.

Only the circle's owner or a caregiver-role account can answer. Signed out, the page sends you to sign in (or to create the account) and brings you straight back. A family-tier member is told plainly that only the owner or a caregiver can answer. If the link was sent to a different email than the one you are signed in with, the page says so and, as the owner, you can still accept.

Once accepted, the practice appears under Organizations following her care on the Care Team page, and the practice's care management and reports are covered by the practice. Ending the enrollment (Manage, then End enrollment) revokes every access the practice had.
