For the family member who runs the care
A care plan app for the family: the real plan, filled from what you already keep
A care plan app for a family should not ask for the plan to be typed twice. ExpressAgain's Home Care Team Plan fills its 25 sections from what the family already keeps in the app, and the day plan, the targets and the Emergency Plan Card are the plan in action.
What is already built
The Home Care Team Plan
The 25 sections of the home care team plan on one page, in the plan document's order, filled from what the family already keeps; two places that disagree show side by side, and every section prints.

The day plan
The patient's real day in time blocks, each with its steps, a how-to video and a checklist; the log keeps who ticked each step and when.

Targets and when to call
The family types the care team's own ranges for glucose, blood pressure, vitals and weight, and the when-to-call instructions; they print beside every log.

The Emergency Plan Card
The home-health agency's patient emergency plan identification card, kept in the app and printable with one button. Every clinical login reads it.

How it works
Create a free account: your name, email, a password and who the app is for (yourself, someone you care for, a patient, or an organization).
The guided setup takes four short steps: who the app is for and the condition mode, text and button sizes, a voice and the first people, then a review.
Tap any phrase on Communicate to say it out loud; invite family with a personal link and give each clinician or professional a code from the Care Team page.
Questions people ask
Is it free to start?
Yes. Free is where everyone starts, and communicating is never behind a paywall: tap to speak, the phrase boards, the sentence builder, Letters, the Phrase Library, lessons, Games and Play, the emergency screen, the Emergency Plan Card and the built-in voice. Free also invites 2 family members to the Family and Friends Feed. The Pricing page shows the plans side by side.
What happens after I sign up?
Sign up asks for your name, email and a password, and who the app is for. Then the guided setup takes four short steps: who the app is for and the condition mode, text and button sizes, a voice and the first people, then a review. Entering the patient's name and date of birth pins the two phrases hospitals ask for constantly.
Can I print the plan?
Yes. Every section of the Home Care Team Plan prints, the day plan prints as a timeline, and the Emergency Plan Card prints with one button.
Can the doctor change the targets?
Yes. A physician or nurse changes a target group from the dashboard and the family's chart follows at once, marked Updated by Dr. and the date, with the old values kept so the family can see the change.
