For the family member who runs the care
Home care after hospital discharge, from the first night
Home care after hospital discharge is hardest in the first week, when the instructions are on paper and the readings have nowhere to go. ExpressAgain's Coming Home checklists get the family ready for the discharge conversation, and the first readings at home go straight to the clinicians against the targets the hospital set.
What is already built
Coming Home
Checklists to walk into the discharge conversation ready, and the therapy team's home recommendations, each marked Done, In progress or Need help.

Targets and care alerts
The family types the targets the care team gave them; every reading becomes one plain line for the clinical logins, and a line outside the targets goes out at once.

Find home care professionals
Search real professionals by the kind of care, the clinician type and a ZIP code, or post an open care request that professionals answer.

The family's side of an enrollment
When a hospital or practice enrolls the patient at discharge, the owner accepts a link that says who wants to follow the patient's care and what they will see.

Today: the Coming Home checklists, readings against the hospital's targets, help from the Marketplace, and the hospital following along if it enrolls the patient.

How it works
Create a free account: your name, email, a password and who the app is for (yourself, someone you care for, a patient, or an organization).
The guided setup takes four short steps: who the app is for and the condition mode, text and button sizes, a voice and the first people, then a review.
Tap any phrase on Communicate to say it out loud; invite family with a personal link and give each clinician or professional a code from the Care Team page.
Questions people ask
Is it free to start?
Yes. Free is where everyone starts, and communicating is never behind a paywall: tap to speak, the phrase boards, the sentence builder, Letters, the Phrase Library, lessons, Games and Play, the emergency screen, the Emergency Plan Card and the built-in voice. Free also invites 2 family members to the Family and Friends Feed. The Pricing page shows the plans side by side.
What happens after I sign up?
Sign up asks for your name, email and a password, and who the app is for. Then the guided setup takes four short steps: who the app is for and the condition mode, text and button sizes, a voice and the first people, then a review. Entering the patient's name and date of birth pins the two phrases hospitals ask for constantly.
Can the hospital follow us at home?
If the physician group enrolls the patient by email or text and the family accepts, the group follows the days at home on its panel and gets the alerts; the family can end it any time.
Can we find an aide for the first weeks?
Yes. Find home care providers searches by the kind of care and a ZIP code, and a questionnaire goes to anyone you are considering with nothing about the patient on it.
