For discharge planners and transitional care teams

Somewhere useful to send a family at discharge

Somewhere useful to send a family at discharge should be more than a pamphlet. ExpressAgain gives the family Coming Home checklists, a day plan, a medication list with reminders and a free tap-to-speak voice, and gives your program a panel that follows the first weeks at home.

What is already built

Coming Home

Checklists to walk into the discharge conversation ready, and the therapy team's home recommendations, each marked Done, In progress or Need help.

Enrollment at discharge

The physician group enrolls the patient by email or text; the invitation names the organization and the practice only, never the patient, and the family accepts.

Transitional care checks

The billing support report checks the discharge on record, the interactive contact within 2 business days and the visit within 7 or 14 days, each row met or not yet.

Admissions and discharge dates on the record

The family can log an emergency room visit, a hospital admission and the discharge date; a typed discharge starts the transitional-care clock and shows who typed it.

Today: Coming Home for the family, enrollment by email or text, and the transitional care checks on Reports.

The organization panel: the stat strip and one row per patient.

How it works

  1. Book the scoping call; a real person walks through your unit, your discharge and your physician groups with your own patients as the example.

  2. Enroll a patient by email or text from the organization panel; the invitation names your organization, never the patient, and the family accepts on the Care Team page.

  3. Follow the days at home: readings against your targets, the dose log, the alerts, and the transitional care checks on Reports.

Questions people ask

Is it free to start?

The family's account is free to start, and Communicate is never behind a paywall. Your organization is set up with our team, starting in preview with a few patients and a few people; the package is set in a call and no price is published on this page.

What happens after I book the call?

A 30-minute call with a real person, using your own patients as the example. Your organization then starts in preview: providers see the patients whose families added them, a few patients can be enrolled to see how it works, and the whole organization comes on when the agreement is signed.

What does the family get for free?

Tap to speak, the phrase boards, the sentence builder, Letters, the Phrase Library, lessons, Games and Play, the emergency screen, the Emergency Plan Card and the built-in voice, plus 2 family invitations to the feed.

Does the program see the medications?

Yes. The dose log and the Medications tab show each dose as given, skipped, refused, missed or late, with who logged it, and a physician or nurse on the team records dose changes.
