For hospitals and health systems

Care that continues after the patient leaves

Care that continues after the patient leaves the hospital needs a record that leaves with the family. ExpressAgain starts at the bedside with a tap-to-speak voice, hands the family the Coming Home plan at discharge, and lets your physician group follow the readings, the medications and the alerts at home.

What is already built

A voice at the bedside

Tap-to-speak on any tablet or phone in the browser, with nothing to install, for a patient who can read and point.

Enrollment at discharge

The physician group enrolls the patient by email or text, and the family accepts; from then on the group follows the days at home on its panel.

The weeks at home, against your targets

The family and the Home Team log readings, meals, doses and falls every day; each reading goes to the clinical logins as an alert line, sent at once when it is outside the targets your team gave the family.

Today: a bedside voice, the ICU mode, enrollment at discharge and the home readings against your targets.

How it works

  1. Book the scoping call; a real person walks through your unit, your discharge and your physician groups with your own patients as the example.

  2. Enroll a patient by email or text from the organization panel; the invitation names your organization, never the patient, and the family accepts on the Care Team page.

  3. Follow the days at home: readings against your targets, the dose log, the alerts, and the transitional care checks on Reports.

Questions people ask

Is it free to start?

The family's account is free to start, and Communicate is never behind a paywall. Your organization is set up with our team, starting in preview with a few patients and a few people; the package is set in a call and no price is published on this page.

What happens after I book the call?

A 30-minute call with a real person, using your own patients as the example. Your organization then starts in preview: providers see the patients whose families added them, a few patients can be enrolled to see how it works, and the whole organization comes on when the agreement is signed.

What does the bedside need?

A tablet or phone with a browser. The guided setup pins the patient's name and date of birth, and the ICU mode opens the breathing-tube and after-surgery phrases first.

Is there an outcomes report?

Outcomes reports in eleven framework types are built from what the family, the professionals and the care team enter, as counts with who logged each; every report reads Available now or Available upon request. No outcome number is claimed.
