For the family member who runs the care

A care coordination app for the home, not the hospital hallway

A care coordination app for a family coordinates the people who actually show up: the aide at 7 AM, the nurse on Tuesday, the speech therapist on video, the daughter who flies in. ExpressAgain gives each of them a login with a role, one schedule, one record, and one feed where the whole team talks.

What is already built

One schedule

One calendar for care shifts, therapy, appointments and visits, with filters, and a booking link an agency uses without a login.

Invite a clinician with a code

Each discipline has its own single-use code: speech, physical and occupational therapy, nursing, physicians, palliative care, hospice and more. Send it by link, email, text or QR.

The IDC Team feed

Video visits

A Telehealth appointment has a video room both sides join from the app: Join video visit appears on the card from 15 minutes before the start, runs in the browser with nothing to install, and nothing is recorded.

Today: one schedule, a code per discipline, the team feed and video visits in the browser.

The Schedule: one calendar for everything, with the booking link for agencies.

How it works

  1. Create a free account: your name, email, a password and who the app is for (yourself, someone you care for, a patient, or an organization).

  2. The guided setup takes four short steps: who the app is for and the condition mode, text and button sizes, a voice and the first people, then a review.

  3. Tap any phrase on Communicate to say it out loud; invite family with a personal link and give each clinician or professional a code from the Care Team page.

Questions people ask

Is it free to start?

Yes. Free is where everyone starts, and communicating is never behind a paywall: tap to speak, the phrase boards, the sentence builder, Letters, the Phrase Library, lessons, Games and Play, the emergency screen, the Emergency Plan Card and the built-in voice. Free also invites 2 family members to the Family and Friends Feed. The Pricing page shows the plans side by side.

What happens after I sign up?

Sign up asks for your name, email and a password, and who the app is for. Then the guided setup takes four short steps: who the app is for and the condition mode, text and button sizes, a voice and the first people, then a review. Entering the patient's name and date of birth pins the two phrases hospitals ask for constantly.

Does the agency see the clinical record?

No. An agency's office login sees Team, Open shifts, Invoices and its agreement, and never the clinical record.

How do clinicians join?

With the family's single-use code for their discipline, at the IDC Team sign-in: the code is checked, they say who they are and choose a password, and from then on email and password work from any device.
