For doctors' offices: read what changed since the last visit
An app for a doctor's office should answer one question before the patient sits down: what changed since the last visit? ExpressAgain sums it up in that card, keeps the home readings against your targets and the dose history you ordered, and runs video visits with a structured note.
What is already built
Since your last visit
The card on the Today door sums up what changed since the clinician's last logged visit: counts, ranges and what is new, never a word that reads meaning into a number.

Targets updated by the doctor, with attribution
A physician or nurse changes a target group from the dashboard and the family's chart follows at once, marked Updated by Dr. and the date.

Video visits and visit notes
A Telehealth appointment has a video room both sides join from the app, in the browser, with nothing recorded; after the visit a structured visit note from the same row.

Epic and eClinicalWorks chart connections
Read-only connections authorized by the patient or their proxy on the hospital's own portal; each one activates on that vendor's approval, and registration is in progress. Until then Doctor's Records says Not set up yet.

Today: Since your last visit, targets with attribution, video visits with a note; the chart connections activate on vendor approval.

How it works
Book the intro call, or ask for a preview on For clinicians; previews are set up personally during early access.
A family sends you a code; at the IDC Team sign-in the code is checked, then you say who you are and choose a password.
Assign exercises from the dashboard; they appear on the patient's Practice page, and Since your last visit sums up what changed.
Questions people ask
Is it free to start?
There is no charge to a clinician for joining a family's circle with the family's code; the family's plan carries the Care Team logins. A practice that follows many patients is set up as an organization on a package in a call, and its pricing page carries no number.
What happens after I book the call?
The intro call is 15 to 20 minutes with a real person, who sets up your clinician preview personally. After that, a family's code at the IDC Team sign-in gives you the care-team dashboard for that one patient; your email and password then work from any device.
Is the EHR link live?
The Epic and eClinicalWorks connections are built; each one activates on that vendor's approval, and registration is in progress. Until then the weekly home summary prints or saves as a PDF for the office.
Can the whole practice follow patients?
Yes. A practice is set up as an organization with a panel of every enrolled patient, care alerts and a billing support report; its package is set in a call.
