For physician practices
For physician practices following patients between visits
For a physician practice, ExpressAgain is better control over patient care while the patient is at home: the home log, care alerts against your targets, and a panel with every enrolled patient. The billing support report documents the care management your staff already provides; it never picks a code.
What is already built
The organization panel
One view of every patient the practice follows: last reading, open alerts and how old, unread feed and enrollment status. Tap a row and the patient's whole dashboard opens.

Targets updated by the doctor
A physician or nurse changes a target group from the dashboard and the family's chart follows at once, marked Updated by Dr. and the date.

Billing support
Whose time counts for which practice, plain requirement checks per care-management program with consent recorded, and the flagged-alert trail. It documents; it never picks a code.

Enrolling patients
Enroll a patient by email or text; the invitation names the organization and the practice only, never the patient, and the family accepts on the Care Team page.

How it works
Book the scoping call; a real person walks through your unit, your discharge and your physician groups with your own patients as the example.
Enroll a patient by email or text from the organization panel; the invitation names your organization, never the patient, and the family accepts on the Care Team page.
Follow the days at home: readings against your targets, the dose log, the alerts, and the transitional care checks on Reports.
Questions people ask
Is it free to start?
The family's account is free to start, and Communicate is never behind a paywall. Your organization is set up with our team, starting in preview with a few patients and a few people; the package is set in a call and no price is published on this page.
What happens after I book the call?
A 30-minute call with a real person, using your own patients as the example. Your organization then starts in preview: providers see the patients whose families added them, a few patients can be enrolled to see how it works, and the whole organization comes on when the agreement is signed.
Does it send a claim?
No. The billing support report keeps the minutes, the contacts and the documentation your billing team reviews; it never sends a claim, never picks a code and never decides what may be billed.
