For families caring for someone with dementia

Caregiver tools for dementia: the checklist, the log and the who-did-what

The tools a family needs for dementia care at home are plain ones: a start-of-shift checklist, a personal care log, a supplies count and a record of who did what. ExpressAgain has each of them, and every entry carries the name of the person who made it.

What is already built

Shift checklists and photo proof

Start-of-shift and end-of-shift lists the family writes, an optional photo before an item counts, and shifts marked as giving medication, with a hard stop for anyone not allowed to.

The personal care log

Showers, bed baths, wash-ups, hair, mouth care, dressing, a skin check and moving around, with how the person moved: steady, needed help, weak, legs gave out, nearly fell or refused.

Household and medical supplies

Wipes, pads, liners and paper towels by scan, picture or typing, counted in packs and pieces, with what was bought and spent this month. Low or Out adds an item to the grocery list.

How it works

  1. Create a free account: your name, email, a password and who the app is for (yourself, someone you care for, a patient, or an organization).

  2. The guided setup takes four short steps: who the app is for and the condition mode, text and button sizes, a voice and the first people, then a review.

  3. Tap any phrase on Communicate to say it out loud; invite family with a personal link and give each clinician or professional a code from the Care Team page.

Questions people ask

Is it free to start?

Yes. Free is where everyone starts, and communicating is never behind a paywall: tap to speak, the phrase boards, the sentence builder, Letters, the Phrase Library, lessons, Games and Play, the emergency screen, the Emergency Plan Card and the built-in voice. Free also invites 2 family members to the Family and Friends Feed. The Pricing page shows the plans side by side.

What happens after I sign up?

Sign up asks for your name, email and a password, and who the app is for. Then the guided setup takes four short steps: who the app is for and the condition mode, text and button sizes, a voice and the first people, then a review. Entering the patient's name and date of birth pins the two phrases hospitals ask for constantly.

Can a shift require a photo?

Yes. With photo proof on, the flagged items on the checklist need a photo from the camera before they count, and a day with required items missing is held for the family's review, never unpaid.

Can I print it for the doctor?

Health reports cover vitals and glucose, medications, symptoms and alerts, the weekly home summary and the visit flow sheet, for 7, 30 or 90 days or a range, with Print and Download CSV.

Gentle, simple and familiar, from the first day.
