Coordinating a home care team: one plan, one record, fewer phone calls

Family, aides, nurses, therapists, doctors: care at home involves a lot of people. How to give them one plan, one schedule and one record, so the family stops repeating the same briefing at every door.

Caring for someone at home quickly turns into running a small organization. There is the family, the aides or nurses on shifts, the home health agency's therapists, the doctors and their offices, the pharmacy, and the neighbor who drives on Thursdays. Every one of them needs to know something, and without a system, the family becomes the switchboard: repeating the same briefing to each new face and answering the same questions by text late at night.

This guide is about building that system. It is written from our founder's own experience of running care at home, and from what ExpressAgain does to carry it.

Two teams, not one

It helps to separate the people into two groups. Our founder's rule: Care Team helps manage my care. Home Team helps me in my home. The Care Team is the doctors, nurses, clinics, therapists and the hospital. The Home Team is the professionals who help in the house: aides, CNAs, LPNs and RNs on shifts, companions and household help. They need different things from you, and mixing them up is how important details get lost. Care Team and Home Team explains the split.

Write the day down

The single most useful document in home care is a written day: when the person wakes, what the morning routine is, when medicines are given, what they eat, how they like things done. Write it as a story a new sitter could follow on their first day, with the steps under each part of the day.

ExpressAgain's day plan shows the day two ways from one source: a Story view that reads like the family wrote it, and a Schedule view, the tight printable timeline an agency expects. Each block can carry a how-to video with exact instructions and a checklist of steps. Whoever does a step ticks it off, and the log keeps who did it and when. The Care Team's dashboard shows the same day, with the same ticks.

Start and end every shift the same way

Handoffs are where things fall through. A short start-of-shift and end-of-shift list solves most of it: check the notes, check the medicines, dishes done, counters wiped, trash out, the next person briefed. In ExpressAgain the family writes both lists, the professional ticks them on their Checklist tab, and the family sees Start 5 of 5, End 4 of 7 with the missed items named. Photo proof is an optional switch for items the family wants to see.

One schedule for everything

Care shifts, therapy visits, doctor's appointments, family visits: if they live in different places, they collide. Keep one calendar. ExpressAgain's schedule holds care shifts, therapy, appointments and visits, with filters, and refuses a booking that overlaps a time already held. A booking link lets an agency reserve an open hour without a login, and taken times show only a label. One chart, logged by whoever is there

Readings, meals, symptoms, falls and doses should be written down once, by the person who saw them, in the place everyone else reads. On ExpressAgain's Health Tracking, every entry carries who logged it, for example Maria (Home health). The Status Board puts today on one screen: the plan, the latest readings with initials and times, and what the team is saying, so the family stops repeating the same briefing at every door.

If the family types in the Care Team's own targets, each reading reaches the clinicians as one plain line, and a reading outside the targets goes out at once. The alert compares a reading with the targets; it never diagnoses.

Keep the whole family informed without more texts

The rest of the family wants to know how the day went, but answering each of them is a job in itself. ExpressAgain's daily note writes up the day every evening from what was logged, by fixed rules, never by a model: a family version, and a gentler friends version that never carries a number, a medication or a symptom. The family reviews it before it posts.

Pay, paperwork and the door

  • Clock in and out: the hours become a timesheet and an invoice the family approves and pays.
  • Pay release is a review, never a forfeiture: a day with a required item missing is held for the family's review, and hours worked are always owed.
  • Agreements, credentials and the W-9 live on each person's profile.
  • Each professional gets their own house entry code, revoked when they leave.

Home Team management covers rates, the weekly schedule, invoices and payment, and every rate and name is typed by the family.

When someone leaves

People move on, and endings go better with a process. A plain notice with the last day, the final invoice and what to bring back, access that ends on the last day and a door code that is revoked: ExpressAgain's offboarding drafts it, and the person moves to Former on the roster rather than disappearing. What is the difference between the Care Team and the Home Team?

Care Team helps manage my care: doctors, nurses, clinics, therapists and the hospital. Home Team helps me in my home: the professionals on shifts, aides, CNAs, LPNs, RNs and companions.

Can an agency's professionals use the same plan?

Yes. An agency's office has its own login for its team, open shifts and invoices, and never sees the clinical record. The people it sends follow the day plan and log under their own names.

Do family members see the medical details?

The owner and caregiver-role accounts see everything. Family members see the day, the chart and the feed but not the medical pages; friends see the friends feed, the Love Bucket and visits.

Does ExpressAgain replace a home health agency?

No. It is where the family, the agency and the professionals share one plan and one record. The care itself is given by the people you choose.
