Patient record, on a patient's row, opens the organization's record of that patient: names, birth and demographics, phones and emails, addresses, language, related people and emergency contacts, care relationships, identifiers (MRN) and, when your organization's sensitive fields are on, insurance and the last 4 of Social Security. Until now it held only what your team typed or imported, so a patient enrolled by invite showed No record details yet.
Now the record fills from the family's account. The moment the family accepts the enrollment, again each time someone on your team opens the patient, and each time the record is loaded, every EMPTY field fills from what the family has entered in ExpressAgain: the patient's first, last and preferred name and date of birth and gender from the profile; the home address, phones, primary caregiver and medical record number from the Emergency Plan Card; the family's own email; the family members on the profile as related people with their relationship; and, only when your organization's sensitive fields are on, the family's insurance as coverage rows carrying the payer, the plan, the group and the LAST 4 of the member ID, never the full number. Nothing the family has not entered appears, and nothing here widens what your organization may read.
Your entries and imports always win. A field your team typed, or a spreadsheet, CCD or FHIR import filled, is never overwritten, and nothing is ever deleted. Each filled row shows where it came from under it (From Sample's ExpressAgain account · Oct 6, with the patient's first name), and when the whole record came from there the Record source line on the row and the panel read the same. An org_audit row names the fields that were filled, never a value.
Update available. When the family later changes one of those values, the next time the record loads it shows an Update available line on that field: The family changed the home address on Oct 8, with Yours and Theirs, and two buttons. Use it writes the family's current value over your copy of that one field; Keep ours keeps yours and does not offer that same change again until the family changes the value once more. Only the people who may edit the record (the patient's providers and administrators) see the buttons; everyone who may read the record sees the line. An archived patient's record never changes.
